反向切取角膜缘干细胞联合应用丝裂霉素C治疗翼状胬肉疗效观察

摘　　要：目的观察应用2．2mm角膜刀反向切取角膜缘干细胞联合应用丝裂霉素C（MMC）治疗翼状胬肉的疗效。方法选取2012年2月至2014年2月在长春爱尔眼科医院手术的260例（260只眼）原发性翼状胬肉患者，在手术显微镜下术中应用2．2mm角膜刀自角膜缘内1mm处向角膜缘方向切取角膜缘干细胞，术中应用0．02％丝裂霉素C浸湿棉片贴敷创面120s，观察术后创面愈合、移植片成活、胬肉复发情况。结果260只眼移植片全部成活，角膜创面愈合良好，术后随访12～24个月，有7只眼复发，复发率2．7％。结论应用2．2mm角膜刀反向切取角膜缘干细胞，移植片切取更快速安全，移植片光滑平整愈合早且良好，应用丝裂霉素C降低复发率，无并发症，安全有效。Objective To observe the effects of backward cutting limbal sterm cell combined with mitomycin C treating pterygium by 2.2 mm keratotome. Methods A total of 260 cases （260 eyes） of primary pterygium patients were collected in our hospital from February 2012 to February 2014. During the surgery, cut from the limbus cornea stern cell 1 mm within the limbus cornea to limbus under microscope, 0.02% MMC cotton patch was put on the surface of the wound for 120 seconds. Corneal wound healing, graft survival, growth and the recurrence of pterygium was observed. Results All 260 eyes of graft survived corneal wound closing well. The patients were received follow-up case for 12-24 monthes, among them 7eyes recurred, and the recurrence rate was 2.7%. Conclusions Backward cutting limbal stem cell by 2.2mm keratotome . The graft operation is rapid and safe. The surface is smooth and neat. The wound close early and well. Applying mitomycin reduce the recurrence and resulte in no postoperative complications. It is safe and effective.
