外科和非外科疗法在外伤性黄斑裂孔患者中的应用效果对比

摘　　要：目的探讨手术与非手术治疗对外伤性黄斑裂孔的治疗效果。方法临床病例对照研究。回颐性分析2010年1月至2015年3月在焦作市人民医院眼科治疗的外伤性黄斑裂孔患者61例61只眼的临床资料。根据患者是否采用手术治疗将患者分为非手术治疗组（27例）和手术治疗组（34例）；非手术治疗组患者给予药物保守治疗，手术治疗组患者采取标准的平坦部玻璃体切割术联合内界膜剥除加14％C3F8气体内加压术治疗。观察两组治疗患者的视力、黄斑裂孔直径的变化及外伤治疗时间等。结果治疗前，两组患者的一般资料均差异无统计学意义（P〉0．05）；手术组裂孔闭合24例（占70．59％），非手术组为11例（40．74％），差异有统计学意义（P〈0．05）；手术组视力提高13例（38．24％），不变或降低21例（61．76％），非手术组视力提高11例（40．74％），不变或降低16例（59．26％），对比结果差异无统计学差异（P〉0．05）；手术组裂孔未闭合患者术后的裂孔大小趋于稳定，有3例视力提高，而非手术组患者的裂孔大小仍不稳定，视力提高5例，两组视力改善情况差异无统计学意义（P〉0．05）。结论对于外伤性黄斑裂孔患者，采用非手术治疗部分患者裂孔可愈合，而手术治疗可提高外伤性黄斑裂孔的愈合率，但视力恢复有限。Objective To investigate the clinical effect of surgical and non-surgical treatment for patients with traumatic macular hole. Methods Clinical case-control form, this study applies retrospective analysis between January 2010 and March 2010 treatment of traumatic macular hole in patients with the clinical data of 61 cases （61 eyes）. According to whether treated by surgery, patients were divided into non-surgical treatment group （27 cases） and surgery treatment group （34 cases）, non-surgical treatment group patients gave medicine conservative treatment and surgical treatment group patients adopted standard fiat of vitreous cutting technique in the joint boundary membrane stripping for the treatment of 14% C3F8 gas pressure. The change of visual acuity, macular hole diameter and trauma treatment time, etc were observed for two groups. Results Before the treatment, no obvious difference was found between two groups of patients＇ general information, no statistically significant （P 〉0.05）. Scores in closed in 24 cases （70.59%）, the scores for 11 cases in （40.74%）, the difference was statistically significant （P 〈0.05）. Visual acuity improved 13 cases （38.24%）, unchanged or decreased 21 cases （61.76%）, and those who did not in 11 cases （40.74%）, the same or lower in 16 cases （59.26%）, the result was not statistically different （P 〉0.05）; Scores in not closed after surgery in size tended to be stable, with 3 cases of vision, rather than the surgical patients in size was still unstable, visual improved in 5 cases, two groups of vision improvement had no significant difference （P 〉0.05）.
