口腔舒合剂对于神经危重症患者口腔溃疡和感染防治的疗效研究

摘　　要：目的分析口腔舒合剂对于神经危重症患者口腔溃疡和感染的防治效果，为临床重症患者口腔溃疡和感染的防治提供新的方法。方法选取2015年7月2016年4月入住某医院神经危重症加强治疗病房住院时间超过4d的患者，2015年7—11月的患者为口腔溃疡和感染的常规防治纽，使用生理盐水进行口腔的常规护理防治；2015年12月-2016年4月的患者为口腔舒合剂防治组，使用按中药组方煎制而成的口腔舒合剂进行防治。均连续5d为一疗程。二组均于入院当天开始每天进行口腔溃疡和感染的临床状态评价。根据患者口腔黏膜有无溃疡、出血或水肿等情况评估口腔护理的效果。结果在收治的患者中常规治疗组74例，口腔舒防治组77例。口腔舒合剂防治组的临床显效率和有效率均明显高于常规防治组（X2=9．657、P=0．008），总有效率也明显高于常规防治组（80．5％、59．5％，X2=8．000、P=0．007）。结论口腔舒合剂对于神经危重症患者口腔溃疡和感染具有较好的防治效果，但应对影响其疗效的相关因素进行更深入地研究。Objective To analyze the clinical efficacy of Kouqiangshu Prescription in preventing canker sore and infection in neurological intensive care unit patients, and to provide the way for clinical medicine. Method Patients whose hospitalization durations were over 4 days with neurological intensive care unit from July 2015 to April 2016 were included into the survey. The patients were as normal control group, from July 2015 to November, using physiological saline for regular oral care prevention and control. The patients were for Kouqiangshu group in December 2015 to April 2016, using according to traditional Chinese medi- cine formula decoction and cure Kouqiangshu for prevention and control. Both of the two groups received medicine 5 days being a therapeutic course. The effect of oral care was assessed according to the conditions that the oral mucosa in patients had ulceration; bleeding or edema etc. Results Patients collected in the schedule time were 74 in the normal control group and 77 patients in the Kouqiangshu group. The signifi- cant efficiency rate and effective rate of Kouqiangshu group was significantly higher than that of normal control group （ X2 = 9. 657,P = 0. 008）. The total effective rate of Kouqiangshu group was significantly higher than the normal control group （80.5%, 59.5%,X2 = 8. 000,P = 0. 007）. Conclusion Kouqiangshu Prescription plays a great control efficiency for qritically ill patient on canker sore and infection prevention. A more in- depth study should be performed at related factors influencing the curative effect.
