彩色多普勒超声对子宫肌瘤与子宫腺肌瘤的鉴别诊断

摘　　要：目的探讨彩色多普勒超声鉴别子宫肌瘤与子宫腺肌瘤的诊断价值。方法采用彩色多普勒超声对103例子宫肌瘤与105例子宫腺肌瘤患者进行检查,对比彩色多普勒超声对两种疾病诊断准确率,同时对比病灶周围血流情况、血流阻力指数(RI)与搏动指数(PI)。结果经阴道超声检查与经腹阴超声联合检查对子宫肌瘤与子宫腺肌瘤的诊断准确率比较均差异无统计学意义(P>0.05);CDFI检查103例子宫肌瘤中75例(72.81%)周边可见环状、半环状血流信号,105例子宫腺肌瘤中仅6例(5.7%),比较差异有统计学意义(P<0.01);子宫腺肌瘤患者RI、PI值均显著高于子宫肌瘤患者,差异具有统计学意义(P<0.01)。结论子宫肌瘤与子宫腺肌瘤超声较难鉴别,经腹超声与经阴道超声联合检查可提高诊断准确率,CDFI测量RI、PI对二种疾病的鉴别诊断具有重要意义。Objective To discuss the application of color Doppler ultrasound in the differential diagnosis of hysteromyoma and endometrioma. Methods One hundred and three patients with hysteromyoma and One hundred and five patients with endometrioma were detected with color Doppler ultrasound. Then the diagnostic accuracy were contrasted between the two groups, and peripheral blood flow,RI,PI were contrasted at the same time. Results There was no significantly difference in diagnostic accuracy of trans abdominal ultrasound and abdominal ultrasound combined with transvaginal ultrasound in the diagnosis of hysteromyo- ma and endometrioma（P〉0.05）. There were circular and half circular flow signals in 75 cases （72.81%） of 103 patients with hysteromyoma and 6 cases （5.7%） of 105 patients with endometrioma（P〈0.01）. RI and PI in patients with hysteromyoma were significantly higher than patients with endometrioma（P〈0.01）. Conclusions Ultrasound has difficult to differentiate hysteromyoma and endometrioma. Abdominal ultrasound combined with transvaginal ultrasound can improve the diagnostic accuracy. The RI and PI meas- ured by CDFI has important significance in the differential diagnosis of two kinds of disease.
