紫杉醇联合顺铂循环式腹腔热灌注化疗对卵巢癌疗效和安全性分析

摘　　要：目的探讨紫杉醇联合顺铂循环式腹腔热灌注化疗对卵巢癌的疗效与安全性。方法回顾性分析我院2011年8月至2013年8月期间收治的96例中晚期卵巢癌患者,根据两种不同治疗方法分为观察组与对照组,每组48例。对照组采用卵巢癌细胞减灭术后予紫杉醇联合铂类全身静脉化疗治疗,观察组采用手术结合紫杉醇联合顺铂循环式腹腔热灌注化疗术,术后予TP方案静脉化疗,对两组患者治疗效果及安全性进行比较。结果观察组治疗有效率为87.50%,对照组治疗有效率为60.42%,两组差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论卵巢癌细胞减灭术后紫杉醇联合顺铂腹腔热灌注化疗辅助静脉化疗对于中晚期卵巢癌患者具有积极的治疗作用,能够有效控制肿瘤进展及恶性腹水,延长患者无进展生存期,提高患者生活质量,且不良反应可耐受。Objective The purpose of this study was to assess the efficacy and safety of hyperthermic intraoperative in- traperitoneal chemotherapy （HIPEC） with Paclitaxel and Cisplatin in the treatment of Ovarian Cancer. Methods A total of 96 patients with advanced ovarian cancer in our hospital between August 2011 and August 2013 were retrospectively analyzed. They were further divided into two groups according to different treatments. Patients in the control group （n = 48） were treated with cytoreductive surgery and post-operative systemic intravenous chemotherapy with paclitaxel and Platinum- based medicines. Patients in the observation group （n=48） were treated with cytoreductive surgery and HIPEC with Paclitaxel and Cisplatin, combined with post-operative intravenous chemotherapy with TP protocol. Efficacy and safety of two groups were compared. Results The treatment effective rate was 87.5% in the observation group, whereas was only 60.42 % in the control group. There was significant difference between the two groups （P〈0.05）. The incidence of adverse effect had no statistical differences between the two groups （P〉0.05）. Conclusion Cytoreductive surgery and HIPEC with Paclitaxel and Cisplatin, combined with post-operative intravenous chemotherapy was effective for patients with advanced ovarian cancer. Such combined treatment could control the progress of tumor and malignant ascites, improve quality of life and progress-free survival.
