摘 要:目的通过完全切断颈后伸肌群的方法,建立针刀治疗的动静力失衡性大鼠颈椎间盘退变模型,为针刀治疗颈椎病提供一种较可靠的实验模型。方法将36只SD大鼠随机分为模型组(18只)、对照组(18只),每组又分1个月、3个月、5个月组,每组6只,模型组采用手术方法完全切断颈后伸肌群,对照组行伪手术。造模术后于相应时间点进行组织形态学观察,并进行免疫组化和定量多聚链反应(PCR)检测Ⅰ、Ⅱ型胶原表达强度及基因表达变化。结果组织形态学观察:颈椎间盘退变程度逐渐加快、加重,模型组颈椎间盘退变较对照组明显;3个月时模型组与对照组比较差异有统计学意义(P<0.05)。Ⅰ、Ⅱ型胶原蛋白检测:(1)免疫组化染色法,Ⅰ型胶原与椎间盘退变呈正相关性,Ⅱ型胶原与椎间盘退变呈负相关性,二者在椎间盘退变过程中发生转换。模型组Ⅰ、Ⅱ型胶原表达均在3个月时较对照组明显(P<0.05),而对照组Ⅰ、Ⅱ型胶原表达5个月较1个月时才开始明显(P<0.05)。(2)PCR检测:随着椎间盘退变进程,Ⅰ型胶原基因表达逐渐增加;Ⅱ型胶原基因表达逐渐减少。模型组Ⅰ、Ⅱ型胶原基因表达均在3个月时较对照组有明显变化(P<0.05)。结论完全切断颈后伸肌群可造成大鼠颈椎动静力失衡,从而加快颈椎间盘退变进程,3个月时即退变明显,为针刀治疗颈椎病的机制研究提供一种理想的动物模型。Objective To establish models of degenerated cervical intervertebral disc in imbalance of dynamic and static forces rats which by means of cut off posterior neck extensors, that provide a stable experimental animal model of Needle - knife on the treatment of cervical spondylopathy. Methods 36 SD rats were randomly divided into model group (n =18 ) and control group ( n = 18) . Then, rats in each group were sub - divided into one - month group, 3 months group and 5 months group, 6 rats in each group. The posterior neck extensors of model group were shut off, and the control group received sham operation. After modeling, the histomorphology at each point in time were observed, and the expressions of immunology and histology chemistry (IHC) and PCR of collagen type Ⅰ and Ⅱ were detected. Results Intervertebral disc degeneration grade was speed up and more serious, the changes in model group were more obviously, and there were statistical differences on intervertebral disc degeneration grade after three months between two groups ( P 〈 0. 05 ) . The IHC of collagen type Ⅰ and Ⅱ : The collagen type Ⅰ was positively correlated with degenerated cervical intervertebral disc, and the collagen type Ⅱ was negative correlation, the transformation of collagen type Ⅰ and Ⅱoccurs during the changes of intervertebral disc degeneration. The expressions of collagen type Ⅰ and Ⅱ after three months in model group were obviously than those in control group ( P 〈0.0 5) . The collagen type Ⅰ and Ⅱ were strongly expressed after five months in control group than those at one month (P 〈0.05). The PCR detections : The expressions of typeⅠ collagen gene was positively related with the process of intervertebral disc degeneration, and the type Ⅰ collagen gene was negative.
