枸橼酸莫沙必利片联合针刺治疗功能性消化不良的临床观察

摘　　要：目的观察枸橼酸莫沙必利片联合针刺治疗功能性消化不良(FD)的临床疗效。方法将136例FD患者随机分为2组,对照组68例予枸橼酸莫沙必利片治疗,治疗组68例在对照组治疗基础上联合针刺治疗。2组均治疗4周,停药4周后随访。观察2组临床疗效,比较2组治疗前后及随访时症状积分,比较2组治疗前后胃排空率、血清胃动素(MTL)、胃促生长素(Ghrelin)及5-羟色胺(5-HT)含量变化。结果治疗组总有效率94.12%,对照组总有效率80.88%,2组总有效率比较差异有统计学意义(P<0.05),治疗组临床疗效优于对照组。2组治疗4周、随访时症状积分均较本组治疗2周降低(P<0.05),且治疗组治疗4周、随访时症状积分较对照组治疗后同期降低明显(P<0.05)。2组治疗后胃排空率均较本组治疗前升高(P<0.05),且治疗组治疗后较对照组升高更明显(P<0.05)。2组治疗后血清MTL、Ghrelin均较本组治疗前升高(P<0.05),5-HT降低(P<0.05);与对照组治疗后比较,治疗组治疗后血清MTL、Ghrelin升高更明显(P<0.05),5-HT降低更明显(P<0.05)。结论枸橼酸莫沙必利片联合针刺治疗FD,能较好改善患者临床症状,提高临床疗效,其机制可能与升高血清MTL、Ghrelin水平,降低5-HT水平有关。Objective To observe the clinical effects of Mosapride Citrate Tablets combined with acupuncture on the treatment of functional dyspepsia （ FD） . Methods 136 FD patients were randomly divided into two groups. 68 cases in control group were treated by Mosapride Citrate Tablets, and 68 cases in treatment group were treated by acupuncture on the basis of control group treatment. The course was four weeks in two groups, and follow -up four weeks after stopping. The curative effects, the symptom scores of before and after treatment and follow - up, the changes of gastric emptying rate, plasma motilin （MTL） , ghrelin and 5 - hydroxytryptamine （5 - HT） werecompared between two groups. Results The total effective rate in treatment group （94. 12% ） was superior to that in control group （ 80. 88% , P 〈 0. 05 ） . The symptom score after treatment and follow - up were decreased in two groups as compared with those of two weeks treatment, and the decrease in treatment group was more obvious than that in control group at the same period （P 〈 0. 05 ）. The gastric emptying rate after treatment in treatment group was increased （P 〈 0. 05 ） , and the increase was significantly higher than control group at the same period （ P 〈 0. 05 ）.The plasma MTL, ghrelin after treatment were increased （ P〈 0 .0 5 ） , and 5 - HT was decreased （P 〈0 . 05） , and the changes of MTL, ghrelin and 5 - HT in treatment group were more significant than those in control group （ P 〈 0. 05 ） . Conclusion Mosapride Citrate Tablets combined with acu-puncture can effectively improve clinical symptoms of patients with FD, and its mechanism may be related to the increase of MTL, ghrelin and the decrease of 5 - HT.
