摘 要:目的探讨脂肪肝超声定量诊断及与肝功能指标和脂肪肝分度生化指标的相关性。方法选取脂肪肝患者163例,根据肝脏氢质子磁共振波谱(1H-MRS)检查结果分为轻度组62例、中度组44例和重度组57例,另选同期健康体检者50例为对照组。行超声定量诊断,并分析回声强度(EI)值与血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、空腹血糖值(FBG)及脂肪肝严重程度的相关性。结果对照组、轻度组、中度组、重度组间左肝、右肝近场、右肝中场EI值及ALT、AST水平呈递增趋势,右肝远场EI值呈递减趋势,差异有统计学意义(P<0.01);ALT水平与左肝、右肝近场、右肝中场EI值呈正相关(r=0.384,P<0.01;r=0.392,P<0.01;r=0.369,P<0.01),与右肝远场EI值呈负相关(r=-0.351,P<0.01);脂肪肝分度与左肝、右肝近场、右肝中场EI值呈显著正相关(r=0.959,P<0.01;r=0.963,P<0.01;r=0.867,P<0.01),与右肝远场EI值呈显著负相关(r=-0.833,P<0.01)。结论超声定量诊断可通过提供量化数据来直接反映脂肪肝病变程度,有助于临床医师判断病情和选择治疗方案。Objective To investigate the application of fatty liver and its correlation with liver function index and grading quantitative ultrasonic diagnosis of of fatty liver. Methods A total of 163 patients with diagnosis of fatty liver were assigned to mild group ( n = 62 ), moderate group ( n = 44) and severe group (n = 57 ) according to the results of proton magnetic resonance spectroscopy (1H-MRS). Another 50 healthy participants were enrolled as the control group. The correlation between echo intensity (EI) and serum level of alanine aminotransferase ( ALT), aspartate aminotrans-ferase (AST), fasting blood glucose (FBG) and grading of fatty liver were analyzed. Results The EI value of left hepatic lobe, right proximal hepatic lobe, right middle hepatic lobe, as well as the level of ALT and AST were increased with the grading of fatty liver ( P 〈 0.01 ).
