摘 要:目的探讨胆管癌患者外周血及肿瘤组织中调节性T细胞(Treg)及相关细胞因子白介素-35(IL-35)的表达及临床意义。方法分别采用流式细胞技术、PCR、酶联免疫吸附法及免疫组化对42例胆管癌患者外周血及肿瘤组织中Treg细胞及IL-35表达进行检测。与正常人比较,分析Treg细胞及IL-35在胆管癌患者中的变化水平及临床意义。结果胆管癌患者外周血中Treg细胞占CD4^T细胞的比例为(5.6±1.7)%,对照组为(2.9±0.8)%,两组差异有统计学意义(P〈0.05)。胆管癌患者外周血浆IL-35水平为(198.4±81.4)pg/ml,对照组IL-35为(33.7±18.0)pg/ml,两组差异有统计学意义(P〈0.05)。外周血单核细胞(PBMC)中,IL-35两个亚基EBI3和p35的mRNA相对表达水平与血浆IL-35表达水平呈正相关(p35,R=0.795,P〈0.05;EBI3,R=0.812,P〈0.05)。免疫组化结果提示胆管癌组织中FOXP3^+癌细胞及Treg细胞明显增多。结论Treg和IL-35在胆管癌患者外周血及肿瘤组织中表达增高,可能与胆管癌的发生发展有关。Objective To investigate the expressions of regulatory T cell (Treg) and interleukin-35 (IL-35) in patients with eholangiocarcinoma and to explore their clinical significance. Methods Flow-eytometry, PCR, Enzyme-linked immunosorbent assay (Elisa) and immunohistoehemistry were used to detect the levels of Treg and IL-35 in peripheral blood and eholangiocarcinoma tissues in 42 patients with cholangiocarcinoma. Healthy volunteers were used as a control group. Result The percentage of Treg cells to CD4 ^+ T cells in patients with cholangiocarcinoma was (5.6 ± 1.7 ) % , while that in the normal control group was ( 2.9 ±0.8 ) %. There was a significant difference between the two groups ( P 〈 0.05 ). The plasma levels of IL-35 in patients with eholangioearcinoma was ( 198.4 ± 81.4) pg/ml, while that in the normal control group was (33.7 ± 18.0) pg/ml. Again, a significant difference was observed between the two groups (P 〈 0. 05 ). In peripheral blood mononuclear cell, the IL-35 mRNA level was positively correlated with the plasma IL-35 level (p35, R = 0.795, P 〈 0.05 ; EBI3, R = 0. 812, P 〈 0.05). Immunohistoehemieal studies showed that FOXP3 + tumor cells and Treg cells increased significantly in tumor tissues. Conclusion Overexpressions of Treg and IL-35 in peripheral blood and tumor tissues of patients with cholangioeareinoma suggested that they may play important roles in the development of cholangiocarcinoma.
