一氧化二氮清醒镇静在上消化道超声内镜检查中的有效性和安全性评价

摘　　要：目的研究一氧化二氮清醒镇静用于内镜超声检查的安全性和有效性。方法以2015-07-01至2015-09-01于中国医科大学附属盛京医院进行内镜超声检查的762例受检者为研究对象,分为一氧化二氮吸入组(观察组,224例),常规检查组(对照组,538例),记录检查过程中患者的血压、心率、血氧饱和度、操作时间、检查后恢复时间及不良反应发生情况,进行统计学分析。结果观察组与对照组均顺利完成内镜超声检查,未出现呼吸抑制、严重心率失常等危及生命的并发症。检查前两组在血压、心率、血氧饱和度等方面差异无统计学意义,检查过程中,观察组的血压、心率显著低于对照组(P均<0.05),而在血氧饱和度方面差异无统计学意义(P>0.05)。观察组在操作时间、检查后恢复时间及不良反应上明显优于对照组(P均<0.05)。结论综合各项指标,一氧化二氮清醒麻醉对于内镜超声检查更有优势,方便易行,不良反应少,安全性高。Objective To study the efficacy and safety of nitrous oxide used during the procedure of endoscopic ultrasound （EUS）. Methods Totally 762 patients who underwent EUS in our hospital were selected, and they were divided into the observation group with 224 cases who were inhaling nitrous oxide, and the control group with 538 cases who underwent only endoscopic ultrasonography. Patient＇s blood pressure, heart rate, blood oxygen saturation, operation time, recovery time after the examination, and adverse events were recorded and analyzed. Results All patients successfully underwent EUS in both observation group and control group without any life-threatening complications such as respiratory depression or severe arrhythmia. There were no difference in blood pressure, heart rate and blood oxygen saturation between two groups before operation. The blood pressure and heart rate had statistically difference between the observe and control groups during the period of operation （P〈0.05）, blood oxygen saturation had no statistical difference （P 〉0.05）. The operation time, recovery time after the inspection, and adverse events had obvious difference in the two groups, the observation group were better than that of control group （all P〈0.05）. Conclusion Awake anesthesia of nitrous oxide for EUS has more advantages, more convenient and feasible, economical and practical, less adverse reaction and high safety.
