摘　　要：目的:分析静息状态下青年女性抑郁症患者自杀行为与脑默认网络(brain default mode network,DMN)功能连接改变的相关性。方法:采用独立成分分析(independent component analysis,ICA)方法,对23例伴有自杀未遂的青年女性抑郁症患者(SD组),10例不伴自杀未遂的青年女性抑郁症患者(NSD组)和31名健康对照者(HC组)进行静息态功能磁共振数据处理,提取DMN,寻找3组组间功能连接差异脑区,并探讨各差异脑区与人口学资料和临床量表评分之间的相关性。结果:与HC组相比,SD组在左侧颞中回(-56,-65,15)、左侧枕中回(-45,-70,18)功能连接增强,NSD组在双侧后扣带回连接增强,2组抑郁症患者双侧楔前叶连接均减弱(校正后P均<0.05);与NSD组相比,SD组左侧舌回(-12,-55,3)、左侧楔前叶(-7,-61,15)功能连接增强,双侧后扣带回功能连接减弱(校正后P均<0.05)。相关分析发现NSD组左侧颞中回时间序列与自杀意念量表评分呈负相关(r=-0.643;P=0.045)。结论:自杀未遂青年女性抑郁症患者存在DMN功能连接异常,其自杀行为可能与楔前叶、后扣带回活性改变有关。Objective：To investigate the correlation between suicidal behavior and brain default mode network（DMN）activity changes at the resting state in young female suffered from depression. Methods：Twenty-three sex-and age-matched female patients with suicide attempt and depression（SD group）were compared with 10 non-suicidal depressed patients（NSD group）and 31 healthy controls（HC group）in the study. Functional connectivity changes in DMN regions which may associate with suicide behavior were explored by comparing resting-state functional magnetic resonance imaging（rs-f MRI）signals using independent component analysis（ICA）approaches. The correlation between time courses of regions of interests（ROIs）in DMN and six clinical scale scores were conducted.Results：Compared with the healthy controls,all depressed patients showed decreased connectivity in bilateral precuneus,suicidal patients showed increased connectivity in the left middle temporal cortex（-56,-65,15）and left middle occipital gyrus（-45,-70,18）while non-suicidal depressed patients showed increased in bilateral posterior cingulate cortex（PCC）（corrected P〈0.05）. Compared with non-suicidal patients,suicidal patients showed increased functional connectivity in the left lingual gyrus（-12,-55,3）,the left precuneus（-7,-61,15）and weakened connectivity in bilateral posterior cingulated cortex（corrected P〈0.05）. Negative correlations between time series of left middle temporal and suicide ideation scale scores（r=-0.643;P=0.045）in non-suicidal depressed patients were revealed. Conclusion：Abnormal brain default network connectivity could emerge in female depressed patients;the suicidal behavior may be associated with abnormal activity in precuneus and posterior cingulate cortex.
