血府逐瘀胶囊联合LED红光照射治疗带状疱疹后遗神经痛疗效观察

摘　　要：目的评价血府逐瘀胶囊联合LED红光照射治疗带状疱疹后遗神经痛(PHN)的临床疗效。方法选取我院近年来收治的80例PHN患者为研究对象,均符合中医学"气滞血瘀型"的临床辨证标准,按照随机平行分组法分为治疗组与对照组各40例。对照组采用LED红光照射、布洛芬缓释胶囊、卡马西平等治疗方案,治疗组在此基础上加用中药血府逐瘀胶囊口服2.4g(6粒),2次/d。连续治疗28d为1个疗程。比较2组治疗前后疼痛评分、睡眠质量及情绪状态等疗效指标。结果治疗1个疗程后,治疗组与对照组临床愈显率分别为77.5%、60.0%,治疗组愈显率明显高于对照组(χ2=9.205,P=0.034);2组治疗前VAS评分、匹兹堡睡眠质量指数(PSQI)及汉密尔顿焦虑量表(HAMA)评分比较并无明显差异(P>0.05),疗程结束当日、疗程结束3个月时上述指标均较治疗前明显降低(P<0.05),但同期组间比较治疗组较对照组降低更显著,差异有统计学意义(P<0.05)。结论血府逐瘀胶囊联合LED红光照射治疗带状疱疹后遗神经痛(PHN)效果理想,可有效减轻疼痛、改善睡眠质量和情绪状态,值得推广应用。Objective To observe the clinical curative effects of Xuefu Zhuyu capsule plus the LED red light irradiation in the treatment of postherpetic neuralgia（PHN）. Methods We selected 80 cases with PHN admitted to our hospital in recent years who met ＂Qi-stagnation and blood stasis＂ of Chinese medical clinical diagnostic criteria as research objectives. All of them were randomly divided into treatment group（n = 40） and control group （n = 40）. Besides LED red light irradiation, ibuprofen sustained release capsule and carbamazepine used for both two groups, the treatment group received Xuefu Zhuyu capsule, 2.4 g（6 pellets）orally two times each day. Continuous treatment for 28 d was regarded as one treatment course. Finally, before and after treatment, we compared pain scores, the sleep quality and emotional state of two groups to evaluate therapeutic efficacy. Results After one-course treatment, the effective rates of the treatment group and control group were 77.5% and 60.0% respectively, which showed that the treatment group got markedly higher rate than the control group（x^2= 9. 205, P= 0. 034）. Before treatment, VAS visual scores, Pittsburgh sleep quality index（PSQI）and the Hamilton anxiety scale（HAMA）scores presented no significant difference between two groups（P〉0.05）, all of which were tremendously decreased on the day of course ending and in the third months（P〈0.05）, simultaneously, compared with the control group, the treatment group had more obvious reduction（P〈0.05）. Conclusion Xuefu Zhuyu capsule combined with the LED red light irradiation may have an ideal effect on postherpetic neuralgia via effectively alleviating pain, improving sleep quality and emotional state, which should be worthy of popularization and application.
