摘 要:目的探讨肿瘤标志物CA125和促甲状腺激素(TSH)联合检测在子宫内膜癌诊断中的应用价值。方法应用电化学发光法和放射免疫法分别测定子宫内膜癌、功能失调性子宫出血患者及健康对照妇女的血清TSH和CA125水平,比较3组间TSH和CA125水平与阳性率的差异,TSH手术治疗前后水平的变化,TSH、CA125联合检测诊断子宫内膜癌的敏感度及特异度。结果 1子宫内膜癌患者血清TSH含量较另外两组明显升高,诊断子宫内膜癌的敏感度为40.00%,特异度为95.00%;子宫内膜癌患者术后血清TSH水平较术前明显下降(P<0.05)。2CA125诊断子宫内膜癌的敏感度为39.67%,特异度为98.33%,较TSH单独检测敏感度低,特异度略高。3TSH和CA125联合检测诊断子宫内膜癌的敏感度为53.33%,特异度为95.00%,较CA125、TSH单项检测敏感度增高,特异度较CA125略低。结论血清TSH检测对子宫内膜癌诊断敏感度高于CA125,TSH与CA125联合检测具有更高的诊断价值;血清TSH还可能作为监测子宫内膜癌治疗效果的指标。Objective To explore the application value of joint detection of CA125 and thyroid stimulating hormone (TSH) in diagnosis of endometrial cancer. Methods Electrochemilumineseence and radioimmunoassay were used to detect serum levels of TSH and CA125 among women with endometrial cancer, women with dysfunctional uterine bleeding (DUB) , and healthy women. The levels and positive rates of serum TSH and CA125, the changes of TSH levels before and after surgery in the three groups were compared; the sensitivity and specificity of joint detection of CA125 and TSH in diagnosis of endometrial cancer were calculated. Results The level of serum TSH in endometrial cancer patients was significantly higher than those in the other two groups, the sensitivity and specificity in diagnosis of endometrial cancer were 40. 00% and 95.00%, respectively. The level of serum TSH after surgery in endometrial cancer patients was statistically significantly lower than that before surgery (P〈0. 05) . The sensitivity and specificity of CA125 in diagnosis of endometrial cancer were 39. 67% and 98.33%, respectively, compared with single detection of TSH, the sensitivity was lower, the specificity was higher. The sensitivity and specificity of joint detection of TSH and CA125 in diagnosis of endometrial cancer were 53. 33% and 95.00%, respectively, compared with single detection of CA125 or TSH, the sensitivity was higher; compared with single detection of CA125, the specificity was lower. Conclusion The sensitivity of serum TSH detection in diagnosis of endometrial cancer is higher than that of serum CA125. Joint detection of TSH and CA125 has higher diagnostic value compared with single detection. Serum TSH can be used as a monitoring index for curative effect of endometrial cancer.
