脑梗死患者用药回顾性分析及临床药学服务模式的探讨

摘　　要：目的:对2013年1月1日至2013年12月31日北京中医药大学第三附属医院脑梗死患者的住院病历进行回顾性分析,寻找临床药师为脑梗死患者提供药学服务的切入点,探讨建立本院脑梗死患者个体化药学服务的工作模式。方法:共纳入脑梗死病例202例,整理患者入院诊断、用药情况、利用EXCEL表格整理计算每种药品使用方法、DDDs和DUI,评价患者选药合理性、联合使用中成药的安全性及合并症用药的相互作用等。结果:该院脑梗死患者治疗用药主要包括以治疗脑梗死为主的抗凝药、抗血小板药和神经保护药,以治疗并发症为目的的降压药、降糖药和调脂药。患者使用药物后不良反应少,依从性较好,但存在用药选择上和临床指南略有差异、中成药使用比例较高和合并症用药较多的问题。结论:通过了解该院当前脑梗死病种治疗的用药情况和存在的问题,根据临床和患者需求初步建立了具有针对性的药学服务模式,进一步提高了药师参与临床药物治疗的工作能力,为优质的药学服务奠定了基础。Objective： In this study, we retrospectively analyzed the hospital records of in-patients with cerebral infarction in our hospital in 2013 in order to identify the point for the clinical pharmacists to provide pharmaceutical care for patients with cerebral infarction, and initially establish the model of individual pharmaceutical care for patients with cerebral infarction in our hospital. Methods： 202 patients with cerebral infarction were selected from all medical records in 2013, and the information such as admission diagnosis and drug use were collected, and the usage amounts of drugs, DDDs and DUI were calculated by EXCEL to analyze the rationality of drug use. Results： The drug use for treating cerebral infarction in our hospital mainly included anticoagulant agents, anti-platelet agents and neuro-protective agents for the purpose of the treatment of cerebral infarction, and anti-hypertensive agents, hypoglycemic agents, and lipid regulating agents for the purpose of treating complications. Observed adverse reactions were few and patients had good compliance. Conclusion： The results showed that drug use for treating cerebral infarction in our hospital was normative. Through this study, clinical pharmacists understand the current situation of the treatment of cerebral infarction in our hospital and the existing problems, and targeted pharmaceutical service model has been initially established according to the clinical and the needs of patients. The ability of pharmacists to participate clinical drug treatment work has been improved in our hospital, thus laying a foundation for the high-quality pharmaceutical care.
