中枢神经细胞瘤及脑室外神经细胞瘤的CT和磁共振成像特征分析

摘　　要：目的探讨中枢神经细胞瘤及脑室外神经细胞瘤的CT及磁共振成像(MR)特征。方法选取2014年9月至2015年3月四川省成都医学院第一附属医院收治的24例神经细胞瘤患者,按部位分为中枢神经细胞瘤(CNC)组和脑室外神经细胞瘤(EVN)组,CNC组15例,EVN组9例。两组患者均行CT、MRI以及病理学检查,分析其影像学和组织学的特点。结果 CT平扫显示,CNC组患者肿块呈稍高密度或等密度,边界清楚,肿瘤密度及囊变等情况因人而异。EVN组患者肿块边界不清,肿块密度及钙化、水肿及出血情况存在个体差异。MRI检查结果显示,CNC组患者T1WI以等信号或略低信号为主,T2WI呈等信号或高低混杂信号为主,肿瘤内部可见囊状的长T1、T2异常信号影,边缘可见血管流空信号。EVN组患者肿瘤以囊性为主,呈脑脊液样信号,少数实性部分呈稍长T1、T2异常信号。病理切片显示,CNC组患者肿瘤组织内细胞形态一致,呈流水样排列,肿瘤细胞内血管丰富,排列紧密。EVN组患者肿瘤组织与少突胶质细胞瘤组织相似,呈均匀一致的蜂窝状小细胞。结论 CNC的CT及MRI征象较典型,EVN常合并水肿和出血,CT和MRI对临床的诊断和鉴别诊断可提供依据。Objective To explore computed tomography（ CT） and magnetic resonance（ MR） features of central neurocytoma and extraventricular neurocytoma. Methods From September 2014 to March2015 in The First Affiliated Hospital of Chengdu,24 patients with neuroblastoma were selected. According to the position of different,they were divided into central neurocytoma（ CNC） group（ n = 15） and extraventricular neurocytoma（ EVN） group（ n = 9）. Computed tomography（ CT）,magnetic resonance imaging（ MR） and pathological examination were explored,and the characteristics of imaging and histology were analyzed. Results CT,MRI scan showed,CNC group of patients with lesions were isodensity or slightly high density,clear boundary,the tumor density and cystic change,differ from man to man,patients in EVN group mass boundary was not clear,there were individual differences in mass density and calcification,edema and bleeding. MRI test results,CNC group T1 WI with isointense or slightly low signal,T2 WI showed ISO signal or mixed signal,T1,long T2 abnormal signal intensity within the visible cystic tumor,the edge of the visible vascular flow void signal,EVN group tumors were cystic,in cerebrospinal fluid sample signal,a solid part slightly long T1,T2 signal abnormalities. Pathology showed,group CNC in tumor tissue cell morphology,flow of water was arranged,the tumor cells arranged closely. rich in blood vessels,tissues and EVN group in tumor tissue was similar,uniform honeycomb cell. Conclusion CT and MRI findings of CNC are typical,and EVN is often associated with edema and hemorrhage,which provides the basis for the diagnosis and differential diagnosis.
