碱性磷酸酶在骨肿瘤诊断中的价值

摘　　要：目的检测不同年龄阶段良、恶性骨肿瘤患者血清中总碱性磷酸酶（TALP）和骨型碱性磷酸酶（BALP）的活性,探讨二者在骨肿瘤的诊断和监测骨肿瘤治疗中的价值。方法选取2010年2月至2015年2月间中国人民解放军济南军区第四五六医院收治的378例良、恶性骨肿瘤患者（肿瘤组）,另选取同期与肿瘤患者对应的不同年龄健康人120例（对照组）,检测血清中的总碱性磷酸酶（TALP）和骨型碱性磷酸酶（BALP）水平。按年龄将肿瘤组和对照组分为〈19岁组、19～50岁组和〉50岁组,每组又按肿瘤性质分为恶性骨肿瘤组、良性骨肿瘤组、转移性骨肿瘤组和对照组,比较各组的血清中TALP和BALP水平差异。结果 〈19岁组中各肿瘤组患者和对照组患者血清中TALP和BALP水平差异无统计学意义（P〉0.05）。19～50岁组中,恶性肿瘤组患者和转移性骨肿瘤组患者血清中TALP和BALP高于良性肿瘤组和对照组,差异有统计学意义（P〈0.05）;〉50岁组中,各肿瘤组患者和对照组患者的血清中TALP和BALP水平差异无统计学意义（P〉0.05）。结论血清中TALP和BALP水平对鉴别诊断19～50岁骨肿瘤患者的良恶性程度有很大帮助;对〈19岁和〉50岁骨肿瘤患者肿瘤性质的诊断作用不大。Objective To exlore the levels of serum levels of malignant bone tumors in serum total alkaline phosphatase（ TALP） and bone type alkaline phosphatase（ BALP） activity,and value of TALP and BALP in identifing and diagnosing of bone tumor. Methods From February 2010 to February 2015 in The PLA Jinan Military 456 Hospital,benign and malignant bone tumors and corresponding to the different age stages of healthy volunteers were divided into three groups according to age, 〈19 yr,19 ～ 50 yr,and〉 50yr. And according to the nature of the tumor in each group,they were divided into malignant bone tumors,benign bone tumor,metastatic bone tumor group and the control group,the serum TALP and BALP levels between the groups were compared. Results 〈19 yr group in various tumor groups and the control group showed no significant difference between serum TALP and BALP（ P〈0. 05）; In 19 ～ 50 yr groups in malignant tumor group and the control group,with statistical significance（ P〈0. 05）;〉 50 yr group in various tumor groups and the control group showed not statisticanct difference between serum TALP and BALP（ P〈0. 05）. Conclusion Serum TALP and BALP determination of 19 ～ 50 yr patients with bone tumor help to diagnoze benign and malignant degree; For 〈19 yr and 〉50 yr patients,they showed no dignostic value.
