血清铁蛋白及4种肿瘤标记物联合检测对老年肺癌患者早期诊断价值分析

摘　　要：目的研究血清铁蛋白(SF)联合癌胚抗原(CEA)、糖类抗原125(CA125)、神经元特异性烯醇化酶(NSE)和血清细胞角蛋白片段21-1(CYRFA21-1)4类肿瘤标记物对老年患者早期肺癌的诊断作用。方法选取2013年7月至2015年6月期间我院收治的150例老年早期肺癌患者作为研究对象,100例肺部良性病变的患者作为对照组,比较2组患者的SF、CEA、CA125、NSE和CYRFA21-1水平。结果 SF联合4种标记物的灵敏度为96.67%,特异度为83.00%,约登指数为0.7967,ROC曲线下面积(AUC)为0.8742;4种肿瘤标记物联合检测的灵敏度为91.33%,特异度为83.00%,约登指数为0.7433,AUC为0.8416,SF联合4种标记物诊断价值高于肿瘤标记物联合检测。结论联合SF检测能够提高4种肿瘤标记物在老年患者早期肺癌诊断中的价值。Objective To study the diagnostic value of serum ferritin （SF） combined with carcinoembryonic antigen （ CEA）, carbohydrate antigen 125 （CA125） , neuronspecific enolase （ NSE）, cytokeratin fragment21-1 （ CYRFA21-1 ） for early lung cancer in the elderly. Methods From July 2013 to June 2015, 150 patients with early lung cancer were en- rolled in this study; at the same time, 100 patients with benign diseases were chosen as control group. The levels of SF, CEA, CA125, NSE and CYRFA21-1 were compared between two groups. The diagnostic values of SF and four tumor mark- ers were analyzed. Results The sensitivity of SF combined with four markers in diagnosing early lung cancer was 96. 67%, the specificity was 83.00%, the Youden＇s index was 0. 7967, the area under ROC curve （AUC） was 0. 8742; the sensitivity of four markers was 91.33%, the specificity was 83.00%, Youden＇s index was 0. 7433 and AUC was 0. 8416. Conclusions SF combined with four markers shows higher diagnostic value for early lung cancer in the elderly.
