阿立哌唑与利培酮对首发未用药精神分裂症患者疗效、认知功能及血清BDNF浓度的影响

摘　　要：目的探讨阿立哌唑与利培酮对首发未用药精神分裂症患者疗效、认知功能及血清脑源性神经营养因子(BDNF)浓度的影响。方法 83例患者随机分为阿立哌唑组(43例)和利培酮组(40例),疗程8周。用阳性和阴性症状量表(PANSS)、精神分裂症认知功能成套测验(MCCB)、副反应量表(TESS)评定临床疗效、认知功能及副反应,用酶联免疫吸附法(ELISA)检测血清BDNF浓度。结果治疗8周后,1两组有效率、副反应发生率差异均无统计学意义(P>0.05);2阿立哌唑组在MCCB中符号编码、语义流畅性、言语记忆、空间广度、数字序列、迷宫、视觉记忆及总分上明显高于利培酮组(P<0.05);3阿立哌唑组血清BDNF浓度明显高于利培酮组(P<0.01)。结论阿立哌唑治疗精神分裂症的总体疗效与利培酮相当;但在改善认知功能、升高血清BDNF浓度优于利培酮。Objective To investigate the influences of aripiprazole and risperidone on clinical efficacy,cognitive function and serum levels of BDNF in patients with drug-naive first-episode schizophrenia. Methods Totally 83 patients were randomly enrolled and divided into aripiprazole group（n=43）and risperidone group（n=40）for 8 therapeutic weeks. Positive and Negative Syndrome Scale（PANSS）,MATRICS Consensus Cognitive Battery（MCCB）and treatment emergent symptom scale（TESS）were used to assess clinical efficacy,cognitive function and adverse reactions,and serum levels of BDNF were measured by enzyme-linked immuno sorbent assay（ELISA）. Results 1 The effective rates and adverse reaction rates had no significant difference in two groups（P0.05）;2 The symbolic coding,category fluency,verbal memory,space width,digit sequence,maze,visual memory and total score of MCCB in aripiprazole group were higher than those in risperidone group（P0.05）;3 Serum levels of BDNF in aripiprazole group were higher than those in risperidone group（P0.05）. Conclusion Aripiprazole and risperidone have similar clinical efficacy in treating first-episode schizophrenia. But aripiprazole improves cognitive function and increases serum levels of BDNF more significantly than risperidone.
