基于社区医院的老年轻度认知障碍的整体治疗

摘　　要：目的探讨药物和多领域认知训练结合的整体治疗对老年轻度认知障碍(MCI)的疗效。方法以唐山市4个卫生服务中心2013年1月至2016年1月收集的195例老年轻度认知障碍患者为研究对象,随机分为药物组(多奈哌齐5mg/晚),干预组(多领域认知训练)和整体治疗组(多奈哌齐5mg/晚+多领域认知训练),每组65例,治疗疗程为12周,干预前后采用可重复的成套神经心理状态测量A套测试手册(RBANS)进行认知功能测评,并比较各组治疗前后认知功能的变化。结果治疗前各组RBANS总分、即刻记忆、空间结构、注意力、言语功能、延迟记忆差异均无统计学意义(P>0.05)。治疗后,各组RBANS总分、即刻记忆、空间结构、注意力、延迟记忆差异均有统计学意义(P<0.05),整体治疗组RBANS总分、即刻记忆、空间结构、延迟记忆均高于药物组和干预组(P<0.05),整体治疗组注意力分数高于药物组(P<0.05)。结论多奈哌齐结合多领域认知训练的整体治疗对老年轻度认知障碍疗效确定,可有效改善其认知功能和日常自理能力。Objective To evaluate the treatment of donepezil combined multi-domain cognitive training in patients with mild cognitive impairment（MCI）. Methods 195 patients with mild cognitive impairment were collected from January 2013 to January 2016 in four community hospitals of Tangshan,which were randomly divided into drug group（donepezil 5mg/night）,the intervention group（multi-domain cognitive training）and the overall treatment group（donepezil 5mg/Night ＋ multi-domain cognitive training）,n=65 cases,treatment duration was 12 weeks. Outcomes were assessed by Repeatable Battery for the Assessment of Neumpsycho Iogiral Status（RBANS）before and after the intervention. The cognitive function before and after treatment were compared. Results The RBANS score,immediate memory,spatial structure,attention,speech function,delayed memory were no significant difference before treatment（P0.05）. After treatment,the differences of RBANS score,immediate memory,spatial structure,attention,memory latency in each group were statistically significant（P0.05）,the RBANS score,immediate memory,spatial structure,are delayed memory in overall treatment group were significantly higher than the drug group and intervention group（P0.05）,the attention in overall treatment group were significantly higher than the drug group（P0.05）.Conclusions Donepezil combined multi-domain cognitive training were effective in treating patients with mild cognitive impairment,which could improve cognitive function and daily living.
