部分脾栓塞在肝癌合并脾亢患者介入治疗中的应用效果观察

摘　　要：目的探讨部分脾栓塞在肝癌合并脾亢患者介入治疗中的应用效果。方法随机选取广州市花都区人民医院2012-05—2015-03间的120例肝癌合并脾亢患者为研究对象,随机分为实验组和对照组,每组患者各60例,实验组采用部分性脾栓塞(PSE)术联合肝动脉化疗栓塞(TACE)术进行治疗,对照组仅实施TACE术,观察两组治疗效果及并发症情况。结果术后1周开始,实验组的WBC、PLT计数明显高于对照组,术后2周、4周仍能保持稳定;而两组的RBC计数则始终未见明显差异。实验组共27例出现并发症(45.0%),明显低于对照组(45例,75.0%),且实验组出现恶心呕吐症状的例数明显少于对照组,差异有统计学意义(P<0.05)。影像学显示均好转。结论部分脾栓塞在肝癌合并脾亢患者介入治疗中的应用效果显著,值得临床使用和推广。Objective To explore the application effect of partial splenic embolization in patients with hepatocellular carcinoma with hypersplenism in interventional therapy. Methods 120 patients of hepatocellular carcinoma with hypersplenism from May,2012 to March,2015 in the hospital were selected randomly as the research object,and they were randomly divided into experimental group and control group,with 60 cases in each group. Patients in the experimental group were treated with PSE combined TACE while the control group were treated with TACE. The effect and complications of two groups were observed. Results 1 week after operation,WBC and PLT in the experimental group was significantly higher than the control group,which can still remain stable from 2 to 4 weeks after the operation; and RBC in two groups has no significant difference. The complications of the experimental group（ 45. 0%） were significantly lower than those in the control group（ 45 persons,75. 0%）,and the number of nausea and vomiting in the experimental group was significantly less than that in the control group. The difference was statistically significant（ P〈0. 05）. Imaging showed improvement. Conclusion Interventional therapeutic effect of partial splenic embolization in hepatocellular carcinoma with hypersplenism in patients is significant,so it is worthy of clinical application and promotion.
