胃息肉193例临床病理及内镜治疗分析

摘　　要：目的：探讨胃息肉内镜临床病理特点。方法：对193例胃镜检查发现息肉住院患者回顾性调查其年龄、性别、息肉的部位、数目、大小以及病理分型,并应用统计学进行统计分析。结果：在2003例胃镜检查中发现息肉193例,息肉检出率为9.64%,女性检出率为14.89%（127/853）高于男性（5.74%,66/1150）。以50-70岁患者为高发人群。胃体部（30.57%,59/193）和胃窦部（25.91%,50/193）是胃息肉的两个好发部位。病理学常见类型依次为炎性息肉（103例,53.37%）、增生性息肉（65例,33.68%）、胃底腺息肉（17例,8.81%）和腺瘤性息肉（8例,4.15%）。息肉的治疗采用高频电凝电切除,氩离子凝固法,尼龙绳结扎法,热活检钳除法,息肉均一次性切除,术后见少量出血,对症治疗出血停止,未见穿孔、感染等并发症。结论：高龄女性是胃息肉高发人群,病理以增生性和炎性息肉常见,手术切除疗效佳。Objective：To investigate the clinical pathological characteristics.Methods：From 2012 to 2015.the cases of gastric polyps diagnosed by gastroscopy were collected.Data of gender,age,location,number,size,pathological type.The data were analyzed by Chisquare test.Results：Among 2003 cases received gastroscopy,gastric polyps were detected in 193 patients and the gastric polyps detection rate was 9.64%,which of female（14.89%,127/853）was higher than that of male（5.74%,66/1150）.And patients aged 50-70years old has a high prevalence of gastric polyps.The positive rate of H.pylori infection in patients detected with gastric polyp was 12.70%（321/2 527）.Gastric body（30.57%,59/193）and gastric antrum（25.91%,50/193）were two predilection sites of gastric polyps.The common pathological types were hyperplastic polyps65（33.68%）,inflammatory polyps103（53.37%）,fundicgland polyps17（8.81%）and adenomatous polyps 8（4.15%）.The treatment of gastric polyps included high frequency electrocoagulation,argon plasma coagulation,and endoscopic mucosal resection.gastric polyps were resected in one time,bleeding occurred after resection treatment to stop bleeding.There was no perforation,infection,and other complications.Conclusion：The detection elderly woman is at high risk.Hyperplastic and inflammatory polyps ware the most common types.Endoscopic resection could achieve a better curative effect.
