血清肿瘤标志物与肺癌肝转移的相关性

摘　　要：目的:探讨血清CEA、CA125、CA19-9、CYFRA21-1和NSE水平与肺癌肝转移发生和预后的相关性。方法:选取华西医院2008年1月至2014年12月Ⅳ期肺癌患者1 135例(发生肝转移171例,其他转移964例)的临床资料,进行资料汇总。结果:肺癌肝转移组中小细胞癌所占比例高于其他转移组,而两组性别、年龄、吸烟量等无差异。CA125、CYFRA21-1和NSE在肺癌肝转移组中的表达水平高于其他转移组,且CYFRA21-1和NSE的阳性率(83.0%,66.7%)高于其他转移组(75.5%,47.9%),有统计学差异(P<0.05)。在肺癌肝转移组中CA125和NSE表达阳性患者中位生存时间均为5个月,表达阴性中位生存时间均为12个月,且差异具有统计学意义(P=0.004,P=0.033)。结论:高表达的血清CA125、CYFRA21-1和NSE水平可能与肺癌患者发生肝转移有关,血清CA125和NSE水平是影响肺癌肝转移患者预后的因素。Objective：To explore the relationship between serum tumor markers,such as CEA,CA125,CA19-9, CYFRA21 -1 and NSE,and the formation and the prognosis of liver metastasis in patients with lung cancer.Meth-ods：The clinical data of 1 135 lung cancer patients hospitalized from January 2008 to December 2014 were retrospec-tively analyzed.All the enrolled patients had pathologic diagnosis.In those patients,171 patients were diagnosed with liver metastasis.The rest had other parts metastasis.Results：The proportion of small cell carcinoma in the group of liver metastasis was higher than the group with other metastasis.The gender,age and cigarette amount had no differ-ence between these two groups.The serum level of CA125,CYFRA21 -1,NSE were higher in liver metastasis group. The positive rates of CYRFA21 -1,NSE were higher in liver metastasis group（83.0%,66.7%vs 75.5%,47.9%,P〈0.05）.The median survival time was shorter among patients with positive CA125 （5 months vs 12 months,P=0.004）and NSE（5 months vs 12 months,P=0.033）than patients with negative CA125 and NSE.Conclusion：The high expression of serum CA125,CYFRA21 -1 and NSE may be related with liver metastasis of lung cancer.The high level of CA125 and NSE may be used as index of the prognosis of liver metastasis in patients with lung cancer.
