摘 要:目的:分析哈尔滨地区不同年龄段妇女人乳头状瘤病毒(human papillomavirus,HPV)的感染状况及基因型分布情况。方法:采用导流杂交基因芯片技术,对17 565例女性的宫颈脱落细胞进行HPV-DNA基因分型,分析各组HPV的总感染率和各亚型感染率及其年龄分布特点。结果:HPV总感染率为20.10%(3 531/17 565)。年龄分层显示,以16-20岁感染率(40.12%)最高,且差异有统计学意义(P〈0.005)。高危型以HPV16感染为主,低危型以HPV11感染为主,中国人常见亚型以HPV53感染为主。3 531例阳性标本中,单一感染率明显较多重感染率高(72.70%vs 27.30%,P〈0.005)。HPV感染者中检出率较高的亚型为16型(23.53%),其次为58型(15.49%)、52型(13.34%)、33型(11.47%)、53型(9.40%)、51型(8.69%)。结论:哈尔滨地区HPV感染具有明显的亚型和年龄分布异质性,且感染年龄趋于年轻化。Objective:To investigate the relationship between subtype HPV infection and different age groups in Harbin,China.Methods:The flow-through hybridization and gene chip technology was used to determine the infec-tion and the genotype of HPV in cervical exfoliated cells for 17 565 women.Results:In 17 565 cases,3 531 cases were detected with HPV-positive infection (20.10%,3 531/17 565).High-risk HPV infection rate appeared a peak at the age of 16-20 by age stratification with statistical difference (P〈0.005).HPV16 was the most common high-risk type,and low-risk type subtype was HPV11.The most Chinese common subtype was HPV53.Single HPV type infection rate was significantly higher than multiple HPV type infection rate (72.70%vs 27.30%,P〈0.005). HPV16(23.53%)was the most common genotype in various types of HPV,followed by HPV58(15.49%),HPV52 (13.34%),HPV33(11.47%),HPV53(9.40%),HPV51(8.69%).Conclusion:The subtype-specific and age-specific distribution of HPV infection in Harbin possesses an obvious heterogeneity,and infection tends to be younger.
