原发性肺腺癌alpha-B-crystallin、TTF-1、NapsinA表达研究

摘　　要：目的分析alpha-B-crystallin、甲状腺转录因子1(TTF-1)、Napsin A在原发性肺腺癌中的表达及意义,为今后该病临床诊断及治疗提供参考。方法收集原发性肺腺癌患者80例作为观察组,将鳞癌、腺癌、腺鳞癌、大细胞癌(共50例)归为对照组;使用免疫组化检测alpha-B-crystallin、TTF-1、Napsin A,分析三指标与原发性肺腺癌病例类型、分期、预后等相关性。结果 alpha-B-crystallin、TTF-1、Napsin A在观察组(肺腺癌)中阳性率分别为20.00%、81.25%、88.75%,与对照组有显著差异(P<0.05);Napsin A、TTF-1、alpha-B-crystallin实性为主阳性率分别为54.20%、65.03%、0;alpha-B-crystallin阳性率在不同肿瘤大小、分期、预后中有显著差异;Napsin A阳性率在不同肿瘤大小、分期、组织分化、淋巴结转移、预后中有显著差异;TTF-1阳性率在不同肿瘤大小、组织分化、分期、预后中有显著差异(P均<0.05)。结论 1TTF-1、Napsin A阳性表达能明确区分原发性肺腺癌、鳞癌、大细胞癌和腺鳞癌,可辅助临床肺癌鉴别;2TTF-1、Napsin A在原发性肺腺癌中有高阳性率,用于肺腺上皮肿瘤病理诊断能提高肺腺癌诊断准确性;3alpha-B-crystallin、TTF-1、Napsin A阳性表达均与肺腺癌病例分期、肿瘤大小、预后相关,对该病预后判断有辅助作用。Objective To explore the significance of expression of alpha - B - crystallin, TTF - 1 and Napsin A in patients with primary lung adenocarcinoma, and to provide the reference for clinical diagnosis and treatment of this disease. Methods 80 patients with primary lung adenocarcinoma were allocated in observation group, and 50 patients with squamous cell carcinoma, adenocarcinoma, adenosquamous carcinoma or large cell carcinoma were listed in control group. Alpha - B - crystallin, TTF - 1 and Napsin A were examined by using immunohistochemical method, and the correlation among results of these three indexes and types of primary lung adenocarcinoma, staging and prognosis of patients had been studied. Results The positive rates of Alpha - B - crystallin, TTF - 1 and Napsin A in patients of observation group （lung adenocarcinoma） were 20. 00% , 81.25% and 88.75% respectively, and the positive rates of NapsinA, TTF - 1 and alpha - B - crystallin in patients of control group were 54. 20% , 65. 03% and 0 respectively. The difference in positive rates of alpha - B - crystallin, tumor size and stages was significant. There was significant difference among prognosis; positive rate of Napsin A and different tumor size, stage, histological differentiation, lymph node metastasis and prognosis. There was significant difference among positive rate of TTF - 1 in different tumor size, differentiation, staging and prognosis （P 〈0.05）. Conclusion （1）The expression of TTF - 1 and Naps i n A can clearly distinguish primary lung adenocarcinoma, squamous cell carcinoma, large cell carcinoma and gland squamous cell carcinoma, and they can assist the clinical identification of lung cancers.（2）There are high positive rates of TTF -1 and NapsinA in patients with primary pulmonary adenocarcinoma, and they can raise the diagnostic accuracy in pathological diagnosis for pulmonary epithelial lung cancer. （3）The positive expression of alpha - B - crystallin, TTF -1 and NapsinA is closely correlated with the stag
