多功能导管固定支架在癌性胸水引流中的应用

摘　　要：目的探讨多功能导管固定支架在癌性胸水引流中的临床应用效果。方法选择2015年1-10月在天津市人民医院肿瘤科进行治疗的晚期癌性胸水患者62例,随机分为观察组和对照组。其中观察组42例,采用多功能导管固定支架进行引流管的固定,引流胸水,同时做好健康教育的宣讲留置管期间的护理;对照组20例,采用传统的中心静脉导管胸腔置管后缝合固定引流管的方法。结果观察组导管出现非计划性拔管现象显著低于对照组,差异有统计学意义(P<0.01);患者舒适度、安全性明显高于对照组,差异有统计学意义(P<0.01);患者离床活动时间明显较对照组延长,差异有统计学意义(P<0.05);感染率明显低于对照组,差异有统计学意义(P<0.01)。结论多功能导管固定支架与皮肤相容性好,保留时间长,易于固定,便于患者离床活动,延长了穿刺间隔时间,有助于降低患者的医疗费用,提高了患者生活质量及满意度,降低了意外脱管后反复穿刺的几率。Objective To discuss the clinical application effect of multi-function catheter fixed bracket in cancerous pleural effusion drainage. Methods From 2015 January to October in Tianjin people＇s hospital for treatment 62 cases of patients with adaneed cancer hydroihorax were randomly divided into observation group and control group. 42 cases of observation group adopted multi-function catheter drainage for drainage of pleural effusion, and at the same time health education were carred out; Control group of 20 cases were given the traditional central venous catheter pleural suture fixation after catheter drainage tube method. Results Catheter in the planned extubation trial phenomenon was significantly lower than control group, the difference was statistically significant （P〈0.01）. Patients with comfort and security significantly higher than the control group, the difference was statistically significant （P〈0.01）. Patients out of the bed activity time obviously longer than control, the difference was statistically significant （P〈0.05）; Infection rates significantly lower than the control group, the difference was statistically significant （P〈0.01）. Conclusion Muhifunctional catheter fixed stent has good compatibility with skin, long retention time and is easy to be fixed, facilitate patients out of bed activity, prolong the time interval between the puncture. It is helpful to reduce the patient＇s medical treatment cost, improve the patient＇s quality of life and satisfaction, reduce the possibility of accident to take off the tube after repeated puncture.
