奥氮平对精神分裂症患者生活质量影响的Meta分析

摘　　要：目的:研究奥氮平对精神分裂症患者生活质量的影响。方法:计算机检索CBM、CNKI和万方数据库,纳入关于奥氮平与其他抗精神病药物影响精神分裂症患者生活质量随机对照试验,选取奥氮平组与非奥氮平组患者的健康状况调查问卷(SF-36)评定数据,应用Revman 5.1软件进行Meta分析。结果:共纳入4项符合标准的研究,合计样本量433例。在生理机能、生理职能、躯体疼痛、总体健康、生命活力、社会功能、情感职能和精神健康维度8方面,治疗前后,奥氮平组患者的效应依次为-12.59、-22.72、-11.64、-7.04、-7.94、-17.26、-23.39、-9.12;95%CI依次为(-17.27,-7.92)、(-35.50,-9.94)、(-16.80,-6.48)、(-11.61,-2.46)、(-11.63,-4.25)、(-21.69,-12.83)、(-37.29,-9.48)、(-12.96,-5.28),差异均有统计学意义。治疗后,与对照组比较,奥氮平组患者效应依次为2.40、7.30、3.42、0.64、-0.80、1.70、4.55、1.37,95%CI依次为(-7.61,12.42)、(-9.21,23.81)、(-1.29,8.13)、(-10.39,4.71)、(-8.84,7.25)、(-4.56,7.95)、(-3.25,12.36)、(-2.23,4.96);差异均无统计学意义(P<0.05)。结论:奥氮平能有效地改善精神分裂症患者的生活质量;奥氮平与其他抗精神病性药物比较略占优势(MD值为正值),但仍需进一步研究证实。Objective To understand effect of Olanzapine on quality of life of patients with schizophrenia. Methods： The CBM, CNK and Wanfang database were searched to collect efficacy data of Olanzapine and other antipsychotics RCTs which affected the life quality of the patients with schizophrenia. Olanzapine group and non Olanzapine group applied health questionnaire （ SF-36） to evaluate, and the data were analyzed by Revman5. 1. Results： Four studies were involved in the Meta- an a lys is, including 433 pa-tients with schizophrenia. From the aspects of physical function, physical ro le, bodily pain , general h e a l th , vitality, social function, e -motional role and mental h e a l th , before and after the treatment, the average effects of olanzapine group were -12. 59, -22. 72 , -11.64, -7.04, -7.94, -17.26, -23.39, -9.12, and 9 5 % Clw re （-17.27, -7.92） , （-35.50, -9.94） , （-16.80, -6.48）,（-11.61, -2.46）, （-11.63, -4.25）, （-21.69, -12.83）, （-37.29, -9.48）, （-12.96, -5.28）, and the differences were statistically significant. Conclu-sions ：It suggests that Olanzapine can significantly improve the quality of life of the patients with schizophrenia. Olanzapine may be slightly superior to other antipsychotic drugs （MD is positive） , and further study is necessary.
