摘 要:目的 研究c erbB 2蛋白表达、基因扩增及其与乳腺癌的关系并将免疫组化 (IHC)和原位杂交 (ISH)结果进行比较。方法 应用免疫组化ABC法检测 14 4例乳腺癌石蜡切片中c erbB 2蛋白表达状况 ,对其中 2 5例浸润型导管癌用光敏生物素标记DNA探针进行原位杂交检测c erbB 2扩增情况。结果 乳腺癌中c erbB 2过度表达阳性率为 4 8 6 % ,浸润型导管癌阳性率最高 ,达 74 1%。过度表达与肿瘤体积、淋巴结转移状况、组织学分级、核分裂计数和临床分期呈正相关 ,与雌激素受体 (ER)、孕激素受体 (PR)状况呈负相关。过度表达者 5年生存率显著低于表达阴性者。 2 5例浸润型导管癌中 ,c erbB 2原位杂交阳性率为 4 8% (12 / 2 5 ) ,与IHC结果一致性 ,IHC强阳性 (3+)者ISH均阳性 (10 / 10 ) ,IHC阴性者ISH均阴性 (8/ 8) ,IHC弱阳性 (1+~ 2 +)者大部分ISH阴性 (5 / 7)。结论 c erbB 2过度表达与乳腺癌不良预后有关。比较IHC和ISH两种方法发现 ,高水平c erbB 2过度表达 (3+)与基因扩增有极好的一致性 。Objective To study c erbB 2 expression and amplification and its relationship with breast cancer, and compare the methods of immunohistochemistry and in situ hybridization on archival breast cancer tissues. Methods A total of 144 breast cancer tissue sections were tested for c erbB 2 overexpression using immunohistochemistry(IHC), of which 25 cases were tested for c erbB 2 amplification using in situ hybridization(ISH) with photo biotin labelled DNA as probe. Results c erbB 2 overexpression was found in 48 6% of overall cases and in 74 1%(the highest) of invasive duct carcinomas. The overexpression correlated with large tumor size, positive nodal status, high clinical stage and histologic grade, more mitotic figures and negative estrogen and progesterone receptor status. The patients with c erbB 2 overexpression had a lower statistically 5 year survival rate than those without overexpression. Using ISH, c erbB 2 amplification was detected in the 25 cases with invasive duct carcinomas. IHC and SHS results were consistant in 80% of cases. All the strongly IHC positive cases were tested ISH positive and all the IHC negative cases were tested ISH negative;large part(5/7) of weakly IHC positive cases were also tested ISH positive. Conclusion c erbB 2 overexpression correlates with poor clinical prognosis. The comparison of ISH and ISH demonstrated an excellent concordance between high level c erbB 2 overexpression and gene amplification.However,weakly positive IHC result (1+~2+) shows only a minor association with gene amplification.
