新辅助化疗联合保乳术治疗Ⅱ、Ⅲ期乳腺癌31例疗效观察

摘　　要：目的探讨乳腺癌Ⅱ、Ⅲ期患者采取保乳术联合新辅助化疗治疗的效果。方法选取Ⅱ、Ⅲ期乳腺癌患者,将其根据治疗方式不同分为对照组与观察组,对照组给予常规根治性切除术+术后辅助化疗,观察组给予新辅助化疗联合保乳术,观察两组疗效。结果两组均随访2年,观察组满意率、住院时间及手术一般指标优于对照组(P<0.05);且观察组并发症发生率低于对照组(P<0.05);观察组乳房外形评估优良率为80.6%。结论新辅助化疗联合保乳术治疗具有微创特点,治疗Ⅱ、Ⅲ期乳腺癌患者并发症少、安全性高,与传统的乳腺癌根治术的近期疗效无明显差异。Objective To study the curative effect of breast conserving surgery and neoadjuvant chemotherapy in the treatment of Stage Ⅱ and Ⅲ of breast cancer. Methods Totally 62 patients with Stage Ⅱ and Ⅲ of breast cancer treated from May 2013 to May 2014 in The First Affiliated Hospital of Guangxi Medical University were selected. The subjects were assigned into control group and test group according to different therapies. The control group adopted the conventional resection and adjuvant chemotherapy while the test group adopted the neoadjuvant chemotherapy and breast conserving surgery. The curative effects for two groups were observed. Results After 2years of follow-up visit,the operation time,hospitalization days and intraoperative bleeding volume for test group was significantly better than those of control group（ P〈0.05）; the incidence rate of complications for test group was significantly lower than that of control group（ P〈0.05）; the satisfaction of test group was higher than that of control group（ P〈0.05）; the recurrence rate and rate of distant metastasis for two groups was not significantly different（ P〈0.05）. According to the breast appearance evaluation results,the test group had 17 good cases,8 excellent cases and 6 poor cases. The acceptance rate for test group was 80. 6%. Conclusion For patients with Stage Ⅱ and Ⅲ of breast cancer,the neoadjuvant chemotherapy and breast conserving surgery is safe,which has less trauma and fewer complications. There is no significant difference in shortterm efficacy between such therapy and conventional resection.
