摘　　要：目的探讨经额中回入路透明套管下神经内镜治疗高血压脑出血的临床效果及其安全性。方法选取本院2012年1月~2015年12月接诊的83例高血压脑出血患者作为研究对象,随机分成观察组（43例）和对照组（40例）。对照组采用传统的开颅手术,观察组采用经额中回入路透明套管下神经内镜治疗。比较两组的临床效果、生活质量评分和并发症发生率。结果观察组的骨窗、切口显著小于对照组,手术时间显著短于对照组,术中出血量和术后血肿清除量显著少于对照组,差异有统计学意义（P〈0.05）。观察组的术后并发症生发生率和死亡率显著低于对照组,术后生活质量评分显著高于对照组,差异有统计学意义（P〈0.05）。结论经额中回入路透明套管下神经内镜治疗高血压脑出血具有较好的临床效果和较低的术后并发症,为高血压脑出血患者开创了新的临床治疗方法,值得推广应用。Objective To explore the clinical therapeutic effect and safety of nerve endoscope with a transparent casing tube in the middle frontal gyrus in the treatment of hypertensive cerebral hemorrhage.Methods 83 cases of hypertensive cerebral hemorrhage in our hospital from January 2012 to December 2015 were selected and randomly divided into the observation group（43 cases） and the control group（40 cases）.The control group was treated with traditional craniotomy,and the observation group was treated with nerve endoscope with a transparent casing tube in the middle frontal gyrus.The clinical effect,quality of life score and the incidence rate of complication was compared between the two groups.Results The bone window and incision of the observation group was less than that of the control group,operation time of the observation group was shorter than that of the control group,the amount of blood loss and postoperative hematoma clearance in the observation group was less than that of the control group,with significant difference（P〈0.05）.The incidence rate of complication and mortality rate of the observation group was lower than that of the control group,the postoperative quality of life score in the observation group was higher than that in the control group,with significant difference（P〈0.05）.Conclusion Nerve endoscope with a transparent casing tube in the middle frontal gyrus in the treatment of hypertensive cerebral hemorrhage has good clinical effect and low postoperative complication.It has created a new clinical treatment method for hypertensive cerebral hemorrhage patients,and it is worthy of promotion and application.
