柴胡疏肝散合四君子汤治疗胃脘痛随机平行对照研究

摘　　要：[目的]观察柴胡疏肝散合四君子汤治疗胃脘痛疗效。[方法]使用随机平行对照方法,将96例门诊患者按就诊顺序号方法简单随机分两组。对照组48例多潘立酮,10mg/次,3次/d;法莫替丁,20mg/次,2次/d。治疗组48例柴胡疏肝散合四君子汤（柴胡、枳壳、香附、川芎、白术各10g,白芍20,陈皮12g,党参、茯苓各15g,炙甘草6g;胃痛严重加元胡、川楝子各10g,理气止痛;泛酸加黄连6g,吴茱萸5g,海螺蛸12g,煅瓦楞子15g,中和胃酸缓解症状;胃脘灼热、口苦口干以疏肝清胃泄热为主,加黄芩10g,蒲公英30g,吴茱萸5g,黄连6g）,1剂/d,水煎300m L,早晚口服。连续治疗30d为1疗程。观测临床症状、不良反应。连续治疗2疗程（60d）,随访1年,判定疗效。[结果]治疗组完全缓解25例,部分缓解12例,稳定8例,进展3例,总有效率93.75%;对照组完全缓解16例,部分缓解11例,稳定7例,进展14例,总有效率70.83%;治疗组疗效优于对照组（P〈0.05）。治疗组有3例复发,复发率12.00%（3/25）,对照组有11例复发,复发率68.75%（11/16）,（χ2=13.9728,P=0.000〈0.01）。[结论]柴胡疏肝散合四君子汤治疗胃脘痛,疗效满意,无严重不良反应,值得推广。[Objective] To observe Chaihu Shugan San Sijunzi Treated epigastric pain efficacy. [Method] Random parallel control method, 96 patients with outpatient treatment order randomly divided into two simple numbers. More than 48 cases in the control group domperidone, 10 mg/time, 3 times/d; famotidine, 20 mg/ times, 2 times/d. Treatment group of 48 patients Chaihu Shugan San Sijunzi Tang （Bupleurum, Citrus aurantium, Cyperus, Chuanxiong, Atractylodes 10 g, white peony root 20, dried tangerine peel 12 g, Codonopsis, Poria each 15 g, Baked Licorice 6 g; severe stomach pain, the Canadian dollar Hu Toosendan the 10g, qi pain; pantothenic acid plus Coptis 6 g, Evodia 5 g, conch octopus 12 g, calcined corrugated sub-15g, neutralize stomach acid to relieve symptoms; epigastric burning, mouth pain, dry mouth to liver Qingwei Xiere based, plus skullcap 10 g, dandelion 30 g, Evodia 5 g, Coptis 6 g）, 1 Ji/d, decoction 300mL, sooner or later orally. 30 d for a course of continuous treatment. Observing clinical symptoms and adverse reactions. Continuous treatment of 2 courses （60 d）, followed up for 1 year to determine efficacy. [Results] Treatment group and 25 cases of complete remission, partial remission in 12 cases, 8 cases were stable, progress in 3 cases, efficiency was 93.75%; control group complete remission in 16 cases, 11 cases were partial remission, 7 cases of stable, progress in 14 cases, efficiency 70.83%. The treatment group curative effect surpasses the control group （P＇〈0.05）. Treatment group, 3 cases of recurrence, recurrence rate of 12.00% （3/25）, the control group 11 cases of recurrence, recurrence rate was 68.75% （11/16）, and （x^2=13.9728, P=0.000〈0.01）. [Conclusion] Chaihu Shugan San Sijunzi Tang Treated epigastric pain, results were satisfactory, no serious adverse reactions, should be promoted.
