小柴胡降逆汤联合针刺治疗胃食管反流随机平行对照研究

摘　　要：[目的]观察小柴胡降逆汤联合针刺治疗胃食管反流疗效。[方法]使用随机平行对照方法,将60例住院患者按病志号抽签法简单随机分两组。对照组30例埃索美拉唑镁,1次/d,1片/次,饭后服。治疗组30例小柴胡降逆汤（党参、柴胡、半夏、白芍、人参、鸡内金各12g,黄芩、黄连、厚朴、生姜、桔梗、陈皮各9g,金钱草30g,竹茹、郁金、麦门冬6g,大枣4枚）,1剂/d,水煎200m L,早晚口服;针刺（足三里、中脘、胃俞、天枢、内关、膏肓、太冲、阳陵泉、期门、上巨虚）,留针20min/次,2d1次。连续治疗2个月为1疗程。观测临床症状、复发率、不良反应。连续治疗1疗程,判定疗效。[结果]治疗组痊愈18例,显效11例,有效0例,无效1例,总有效率96.67%;对照组痊愈11例,显效10例,有效0例,无效9例,总有效率70.00%;治疗组疗效优于对照组（P〈0.01）。随访6个月,治疗组复发2例（11.11%,2/18）,对照组复发7例（63.64%,7/11）,治疗组低于对照组（χ^2=10.88,P〈0.05）。不良反应对照组7例,头痛3例,腹泻2例,腹胀1例,恶心呕吐1例,治疗组4例恶心呕吐2例,口干1例,头晕1例,经过减低药量或停药处理后,未影响治疗效果,继续观察。[结论]小柴胡降逆汤联合针刺治疗胃食管反流,疗效满意,无严重不良反应,值得推广。[Objective] To observe Xiaochaihu Jiangni Tang combined with acupuncture treatment of gastroesophageal reflux effect. [Method] Random parallel control method, 60 cases of hospitalized patients by disease Zhi number lottery method is simple random divided into two groups. 30 patients esomeprazole magnesium, 1 times/d, 1/ times after meals. Treatment group of 30 patients Xiaoehaihu Jiangni soup （Codonopsis, Radix, Pinellia, white peony root, ginseng, Gallus gallus domestieus 12 g~ Scutellaria, Coptis, Magnolia, ginger, Campanulaeeae, dried tangerine peel 9 g, money grass 30 g, Zhuru, turmeric, wheat Asp 6g, jujube 4）, l/d, decoction 200 mL, sooner or later orally; acupuncture （Zusanli, abdominal, stomach Yu, Tianshu, the customs, Gaohuangshu, Taichong, Yanglingquan, on the door, on the giant virtual）, the needle 20 min/times, 2 dl times. Continuous treatment of 2 months for a course of treatment. Observation of clinical symptoms, the recurrence rate of adverse reactions. Continuous treatment course of treatment, to determine efficacy. [Results] The clinical efficacy of the treatment group than the control group （P〈0.01）. 6-month follow-up, the treatment group relapsed 2 cases （11.11%, 2/18）, 7 cases of recurrence in the control group （63.64%,7/11）, the treatment group than the control group （x^2=10.88, P〈0.05）. Adverse reactions in the control group, 7 cases of headache in 3 cases, 2 cases of diarrhea, abdominal distension one case, one case of nausea and vomiting. The treatment group d cases of nausea and vomiting in 2 cases, 1 case of dry mouth, dizziness, 1 case after dose reduction or discontinuation of treatment, no effective treatment, continue to observe. [Conclusion] Xiaochaihu Jiangni decoction combined with acupuncture treatment of gastroesophageal reflux, results were satisfactory, no serious adverse reactions, should be promoted.
