创伤性唇颊缺损畸形修复126例

摘　　要：目的　研究应用各种皮瓣、皮管、真皮下血管网皮片及全厚皮肤移植 ,修复唇颊缺损畸形。方法　依据唇颊缺损的面积和损伤的深度 ,选择相应的唇交叉瓣、鼻唇沟皮瓣、前臂桡动脉游离皮瓣、上臂皮管、真皮下血管网皮片和全厚皮片移植。结果　治疗 12 6例 ,Ⅰ期手术完成 98例 ,Ⅱ～Ⅳ期手术完成 2 8例。各类皮瓣、皮管、皮肤移植全部成活 ,4例修复上唇仍有轻度外翻 ,经再次手术修复后恢复自然状态。结论　创伤性唇颊缺损是以缺损的程度选择修复的方法 ,修复得当 ,可使唇的功能外观同时恢复对称自然美 ,使皮瓣、皮管的表面色泽与受区相近 ,但皮肤移植者色泽相差较为明显。Objective To study the clnical treatment for defects and deformities in lip buccal region by using different skin flap, skin tube, free skin graft with subdermal vascular plexus and full thickness skin graft. Methods According to the depth and the area of defects, appropriate measurements were employed, which included cross flap of the lip, nasolabial fold skin flap, foreaem free skin flap, free skin graft with subdermal vascular plexus and full thickness skin graft. Results Of 126 cases, 98 cases were cured in the one stage operation, and 28 cases in the two to four stage operation. All kinds of flaps, skin tube and skin grafts survived completely. After correction, 4 cases with mild upper lip ectropion were recovered. Conclusion The repair methods will be choosed according to the degree of defects. If suitable measurements are taken, the function and the apperance of the lip can restore to a natrural condition at the same time. Comparing the color and lustre with normal skin, there is little difference after transplantion of flaps and skin tuge while obvious difference can be obsenrved with skin grafting.
