肺泡灌洗液中葡萄糖、腺苷脱氨酶、癌胚抗原检测在肺癌中的诊断价值

摘　　要：目的评估肺泡灌洗液(BALF)中葡萄糖(GLU)、腺苷脱氨酶(ADA)、癌胚抗原(CEA)在肺癌中的诊断价值,根据统计学方法制定临床诊断肺癌的可能截断值。方法定量检测分析100例肺部阴影患者肺泡灌洗液中GLU、ADA、CEA浓度;ADA采用生化分析仪速率法检测,CEA采用时间分辨仪时间分辨法检测,GLU采用生化分析仪己糖乙酶法检测。同时行肺活检或黏膜活检、刷检等病理学或细胞学检查。结果100例患者确诊肺癌64例,非肺癌36例;两组患者BALF中GLU检测比较差异无统计学意义(P>0.05);CEA/GLU≥262.4时诊断肺癌的敏感性为95%,特异性为64.7%;CEA≥7.87ng/m L时诊断肺癌的敏感性为85%,特异性为58.8%。结论 BALF中GLU、CEA检测对肺癌的早诊断有一定指导价值,当CEA/GLU≥262.4,CEA≥7.87ng/m L时,临床诊断肺癌的可能性较大。Objective To investigate the diagnostic value of glucose（ GLU）,adenosine deaminase（ ADA）and carcinoembryonic antigen（ CEA） in bronchoalveolar lavage fluid（ BALF） of patients with lung cancer. Methods GLU,ADA and CEA in BALF of 100 patients with pulmonary shadow were quantitatively detected： ADA was detected by biochemical analyzer and rate assay,CEA was detected with time-resolved method by time-resolved spectrometer,and GLU was detected with hexokinase method by biochemical analyzer. At the same time,lung biopsy or mucous membrane biopsy,and brush biopsy were carried out. Results 64 cases were proved to be malignant and 36 benign. Between the two groups,GLU detection showed no statistical difference（ P〉0. 05）,and CEA in BALF of patients of lung cancer was significantly increased（ P〈0. 05）. Combined with CEA / GLU≥262. 4,the sensitivity and specificity to diagnose lung cancer was 95% and 64. 7%. Combined with CEA≥7. 87 ng / m L,the sensitivity and specificity were 85% and 58. 8%. Conclusion GLU and CEA detection of BALF may have a certain diagnostic value of lung cancer. When CEA / GLU is greater than or equal to 262. 4 and CEA is greater than or equal to 7. 87 ng /m L,the possibility of clinical diagnosis of lung cancer is relatively large.
