葡萄膜炎并发白内障患者术中前房注射地塞米松的临床疗效

摘　　要：目的探讨葡萄膜炎并发白内障患者术中前房注射地塞米松的临床疗效。方法选取2013年5月至2015年2月哈尔滨理工大学医院收治的葡萄膜炎并发白内障患者共63例,根据随机、单盲、平行对照设计原则将其分为对照组（31例）和观察组（32例）。对照组患者采用白内障超声乳化治疗联合人工晶体植入术,而观察组患者则在手术过程中在前房注射地塞米松,比较两组患者的视力改善程度、角膜水肿程度、前房闪辉3-4级比例、眼压。结果观察组患者视力相较于治疗前提高比例明显高于对照组,差异有统计学意义（P〈0.05）;观察组角膜无水肿患者比例显著高于对照组,差异有统计学意义（P〈0.05）;观察组术后第1天、第2天前房闪辉为3-4级患者比例显著低于对照组,差异均有统计学意义（均P〈0.05）。结论葡萄膜炎并发白内障患者的治疗过程中,前房注射地塞米松能够有效提高患者视力,并能改善角膜水肿情况,且不会对眼压造成影响,安全可靠。Objective To investigate the clinical effect of dexamethasone into anterior uveitis complicated cataract surgery.Methods Selection may 2013 to 2015 February Harbin Science University Hospital of uveitis complicated cataract patients with a total of 63 cases,according to a randomized,single blind,parallel control design principles can be divided into the control group（31 cases） and observation group（32 cases）.The control group of patients with cataractphacoemulsification combined with intraocular lens implantation in the treatment of,and the patients in the observation group is in the process of operation in the anterior chamber injection of dexamethasone,compared two groups of patients with visual acuity improvement,corneal edema,anterior chamber flare 3~4 grade proportion, intraocular pressure（IOP）.Results Observation group,visual acuity is compared with before treatment increased the proportion of patients was significantly higher than that of the control group,the difference is statistically significant（P0.05）; observation group corneal edema in the proportion of patients was significantly higher than that of the control group, the difference is statistically significant（P〈0.05）;patients in the observation group after the 1 D,2 D anterior chamber flare for 3-ratio of level 4 patients was significantly lower than that of the control group,the difference is statistically significant（P〈0.05）.Conclusion In the treatment of uveitis complicated cataract,anterior chamber injection of dexamethasone can improve the vision of patients,and improve the state of corneal edema,and not on intraocular pressure（IOP） impact,safe and reliable.
