蛛网膜下腔出血患者Hunt-Hess分级与血清脑钠肽的关系

摘　　要：目的 探讨动脉瘤性蛛网膜下腔出血患者Hunt-Hess分级与血清脑钠肽（BNP）的关系.方法 选取我院收治的经脑血管造影证实为动脉瘤性蛛网膜下腔出血患者45例,其中Hunt-Hess Ⅰ级5例,Ⅱ级12例,Ⅲ级15例,Ⅳ级8例,V级5例,所有患者均在蛛网膜下腔出血后48 h内入院.于发病后第3天、1、2周时检测血清BNP水平.比较Hunt-Hess同级患者不同测定时间点血清BNP水平和相同测定时间点不同Hunt-Hess分级患者血清BNP水平.结果 发病后第3天、1、2周3个时间点的血清BNP水平分别为：Hunt-Hess Ⅰ级：（32.34±12.89）、（33.97±11.56）、（30.94±10.20） pg/ml,差异无统计学意义（P＞0.05）;Hunt-HessⅡ级：（33.48±11.96）、（34.57±12.01）、（29.78±11.73） pg/ml,差异无统计学意义（P＞0.05）;Hunt-HessⅢ级：（49.87±19.16）、（89.57±24.36）、（51.45±20.67） pg/ml,差异有统计学意义（P＜0.05）;Hunt-HessⅣ级：（79.46±23.48）、（125.72±39.78）、（81.71±25.45） pg/ml,差异有统计学意义（P＜0.05）;Hunt-HessV级：（91.12±29.46）、（153.84±50.96）、（90.87±31.49） pg/ml,差异有统计学意义（P＜0.05）.相同检测时间点,Hunt-HessⅢ-V级患者血清BNP水平较Hunt-Hess I-Ⅱ级患者明显升高（P＜0.05）.结论 动脉瘤性蛛网膜下腔出血患者血清BNP水平与Hunt-Hess分级呈正相关。Objective To investigate relationship between Hunt-Hess score in patients suffering from aneurysmal subarachnoid hemorrhage （SAH） and plasma brain natriuretic peptide （BNP） concentrations.Methods Forty-five patients admitted because of spontaneous SAH who confirmed by cerebral angiography at the Department of Neurosurgery,Henan Provincial People＆#39; s Hospital were recruited for the study.All patients were admitted within 48 h after the onset of SAH.Neurological status was assessed according to the Hunt-Hess classification：Hunt-Hess score Ⅰ （n =5）,Hunt-Hess score Ⅱ （n =12）,Hunt-Hess score Ⅲ （n =15）,Hunt-Hess score Ⅳ（n =8）,and Hunt-Hess score V （n =5）.Plasma BNP concentrations were assessed at three different time periods （3rd day,1 st week,and 2nd week）after the onset in patients with spontaneous SAH.Plasma BNP levels in patients with identical Hunt-Hess score at three different time periods,and those with distinct Hunt-Hess score at the same time period were compared.Results At three different time periods （3rd day,1 st week,and 2nd week） after the onset,plasma BNP levels in patients with Hunt-Hess score Ⅰ were （32.34 ± 12.89）,（33.97 ± 11.56）,and （30.94 ± 10.20） pg/ml （P 〉 0.05）,those in patients with Hunt-Hess score Ⅱ were （33.48 ± 11.96）,（34.57 ± 12.01）,and （29.78 ± 11.73） pg/ml （P 〉 0.05）,those in patients with Hunt-Hess score Ⅲ were （49.87 ± 19.16）,（89.57：± 24.36）,and （51.45：± 20.67） pg/ml （P 〈 0.05）,those in patients with Hunt-Hess score IV were （79.46 ±23.48）,125.72 ±39.78）,and （81.71 ±25.45） pg/ml （P〈 0.05）,and those in patients with Hunt-Hess score V were （91.12 ±29.46）,（153.84 ±50.96）,and （90.87 ±31.49） pg/ml （P〈 0.05）.At the same time period,plasma BNP levels in patients with Hunt-Hess score Ⅲ-V were significantly elevated as compared with those in patients with Hunt-Hess score Ⅰ-Ⅱ （P 〈 0.05）.Conclusion The plasma BNP levels are po
