摘  要:目的:探讨食物因素与溃疡性结肠炎( ulcerative colitis, UC)的临床相关性及机制。方法对30例UC患者和30例对照进行血清学和组织细胞学检测,酶联免疫法分析血清中食物特异性IgG、食物特异性IgE和相关细胞因子水平,免疫组化分析UC肠粘膜IgG免疫复合物水平、肥大细胞与巨噬细胞浸润。结果 UC组血清中食物特异性IgG阳性21例,阳性率为70%,显著高于对照组的43.3%, UC组血清中TNF-α、 IL-6水平也高于对照组。血清中食物特异性IgE、 IL-4、 IL-5在两组之间无显著性差异。 UC组肠粘膜中IgG、 CD68、 tryptase免疫组化阳性程度高。结论食物特异性IgG与溃疡性结肠炎发病显著相关,其免疫复合物在肠粘膜的沉积、肥大细胞与巨噬细胞在肠粘膜的浸润与活化在该过程中发挥重要作用。Objective To examine the clinical correlation between dietary factors and ulcera-tive colitis ( UC) and the mechanism.Methods A total of 30 UC patients and 30 control subjects were subjected to serological and histocytological examinations.Levels of food-specific IgG, food-specific IgE and related cytokines were detected in the serum by enzyme-linked immunoassay.Levels of immune complexes, mast cells and macrophages infiltration were measured by immunohistochemi-cal analysis in colonic mucosa of UC tissues.Results The positive rate of serum food-specific IgG in the UC group was 70% ( n =21 ) , significant higher than that in control group ( 43.3%) .Inflammatory cytokines IL-6 and TNF-αin the serum were higher in the UC group than in the con-trol group.There were no significant differences in the levels of food-specific IgE, IL-4 and IL-5 in the serum between the two groups.Immunohistochemical analysis revealed increased expression of IgG, CD68 and tryptase in the intestinal mucosa in the UC group.Conclusion Food-specific IgG is significantly associated with the development of UC.The deposition of immune complexes and the infiltration and activation of mast cells and macrophages in the intestinal mucosa play important roles in UC.
