Self-reported professional practice and cognition related to doctor-patient relationship among clinical doctors

摘  要:目的了解目前医患关系中医生群体的职业精神现状,为建立和谐医患关系提供参考。方法 2014年8月,采用分层随机抽样法,抽取广东省广州市35家医院的1 320名医生进行问卷调查,使用SPSS 17.0软件对数据进行分析。结果 78.10%的医生认为目前医患关系一般及不太好;47.30%的医生能做到依法行医;在医疗质量、人文关怀、医患沟通、伦理关系、医疗方式、心理交流、情绪控制方面表现良好的分别占80.00%、74.40%、85.00%、78.50%、70.10%、75.00%、72.35%;认为医患关系紧张主要责任在医患双方的占58.90%;54.40%的医生曾遇侵医事件;有过度医疗的占29.17%,有自卫性医疗的占43.33%;考虑经济问题较多的占66.52%;高级职称组总均分、依法行医、医疗服务质量、人文关怀维度得分分别为(3.89±0.92)、(3.23±1.06)、(3.96±0.79)、(3.68±0.78)分,初、中级职称组分别为(3.59±0.95)、(2.37±0.76)、(3.58±0.73)、(3.40±0.73)分,差异均有统计学意义(均P<0.05);非手术组总均分、医患沟通、伦理关系、医疗方式、心理交流、情绪控制维度得分分别为(3.82±0.94)、(4.24±0.63)、(4.13±0.67)、(2.87±0.83)、(3.78±0.58)、(3.77±0.68)分,手术组得分分别为(3.56±0.91)、(4.01±0.56)、(3.92±0.65)、(2.66±0.83)、(3.42±0.80)、(3.13±0.86)分,差异均有统计学意义。结论医生群体对医患关系总评价偏低,职业精神存在消极、自卫现象,有一定程度物化;大部分医生能尽心尽力为病人服务,素质高、医疗时间充裕的医生能获得更好的医患关系。Objective To explore the status quo of doctor-patient relationship (DPR) and its related professional practice from the view point of clinical doctors and to provide evidences for establishing harmonious DPR. Methods A face-to-face interview with a self-designed questionnaire was conducted among 1 320 clinical doctors selected from 32 grade II or III hospitals in Guangdong province with stratified random sampling in August 2014. SPSS 17.0 was used in data analyses. Results Of the doctors surveyed,78.10% considered that DPR was at a general level and 47.30% evaluated themselve's medical service being in accordance with relevant laws and regulations. The ratios of the doctors reporting good personal professional practice were 80. 00% in qualified medical service ,74. 40% in humanistic care,85.00% in doctor-patient communication,78.50% in ethnic management,70. 10% in therapeutic suggestion,75.00% in psychological communication, and 72. 35 % in emotion control, respectively. There were 58.90% of the doctors considering that both doctors and patients are responsible for the stressed DPR, 54. 40% reporting the experience of aggression towards medical staff,29. 90% reporting excessive medication ,43.30 reporting medication for the purpose of self-protection, and 66. 50% reporting concerns on economic issues. The doctors with senior professional tiles awarded significantly higher scores for overall DPR ( 3.89 ± 0. 92 vs. 3.59 ± 0. 95 ), medical service in accordance with relevant laws and regulations (3.23 ± 1.06 vs. 2. 37 ±0. 76) ,quality of medical service (3.96 ±0. 79 vs. 3.58 ±0. 73) ,and humanistic care (3.68 ± 0. 78 vs. 3.40 ±0. 73) compared to the doctors with junior or intermediate titles (P 〈0. 05 for all) ;compared to the doctors working in surgery or obstetrics, the doctors working in other departments awarded significantly higher scores for overall DPR (3.82 ±0. 94 vs. 3.56 ±0. 91) ,doctor-patient communication (4. 24 ±0. 63 vs. 4. 01 ±0. 56) ,ethnic management
