CT导引下经皮穿刺氩氦冷冻治疗高龄周围型肺癌的临床应用

摘　　要：目的探讨CT引导下氩氦冷冻治疗高龄周围型肺癌的安全性、可行性及近期疗效。方法对19例高龄周围型肺癌患者行CT引导下氩氦冷冻治疗,随访观察治疗后3、6、12、24个月时肿瘤变化,统计肿瘤局部控制率和患者生存率。结果治疗后3、6、12、24个月病灶控制有效率(RR)分别为:100%(19/19)、94.7%(18/19)、89.5%(17/19)、84.2%(16/19);19例患者均顺利完成手术,手术过程安全,未出现严重并发症,其中气胸1例,少量咯血或痰中带血2例,胸腔积液1例,术后发热3例;治疗后3、6、12、24个月患者生存率分别为100%(19/19)、100%(19/19)、89.5%(17/19)、84.2%(16/19)。结论 CT导引下经皮穿刺氩氦冷冻治疗高龄周围型肺癌,操作安全,并发症少,近期疗效可靠,可重复进行。Objective To investigate the safety,feasibility and short-term efficacy of CT guided argon helium refrigeration in the treatment of elderly patients with peripheral lung cancer.Methods 19 cases of elderly patients with peripheral lung cancer were treated with CT guided argon helium freezing treatment,follow up at 3,6,12 and 24 months was performed for evaluation of tumor changes,statistical tumor local control rate and survival rate of patients.Results at 3,6,12 and 24 months after treatment,the lesions were 100%（19/19）,94.7%（18/19）,89.5%（17/19）,84.2%（16/19）,respectively;19 patients were successfully operated with no serious complications,there were 2 cases of pneumothorax,1 case of hemoptysis,3 cases of pleural effusion.The survival rates at 3,6,12 and 24 months after treatment were100%（19/19） 、89.5%（17/19） 、84.2%（16/19）,respectively.Conclusion CT guided percutaneous argon helium refrigeration for the treatment of elderly patients with lung cancer is safe and with less complications.the short-term efficacy is reliable and can be repeated.
