摘　　要：目的探讨应用25G玻璃体切割系统治疗婴幼儿先天性白内障的临床疗效。方法回顾性分析我院13例（21眼）婴幼儿先天性白内障患儿在全身麻醉下，应用25G玻璃体切割系统在角巩膜缘做切口行前囊膜环形切除＋白内障吸出＋后囊膜环形切除＋前段玻璃体切割术。术后随访4～20个月，平均8个月，观察患儿视力、眼压、前房反应及并发症的发生情况。结果所有手术均顺利完成，术中前房稳定，无明显虹膜脱出，术后视力均明显改善，表现为明显追光、追物。术后眼压均在正常范围内，除2眼术后第1天发生低眼压（术后第3天恢复正常），所有患儿术后角膜清亮，前房无出血、渗出。随访期间所有患儿视轴区透明，无明显并发症发生。结论25G玻璃体切割系统用于治疗婴幼儿先天性白内障手术创伤小，对眼内组织刺激小，术后炎症反应轻、并发症少，手术安全有效。Objective To observe the clinical effects of application of 25G vitreous surgery system in treating congenital cataract of infant. Methods This study was a retrospective analysis. 25G vitreous surgery system was applied to 13 infant patients （21 eyes） with congenital cataract, lensectomy combined with anterior and posterior capsular resection with anterior vitrectomy under general anesthesia were performed. The average follow-up period was 8 months （ ranged from 4 months to 20 months）, the visual acuity, intraocular pressure and complications during and after the operation were observed. Results All operations were performed successfully. There was no complications during the perioperative follow-up period, such as dislocation or iris prolapse. Postoperative visual acuity were markedly improved, obviously showed the spotlight, after things, except that 2 eyes occurred temporary low intraocular pressure （ after 3 days resumed normal）, the rest eye with the normal intraocular pressure in follow-up period. All the corneas were clear and there was no hyphema and effusion. All cases obtained transparent visual area, there was no obvious complication in follow-up period. Conclusion It is safe and effective in applying Vitreetomy 25G system to treat the congenital cataract of infant with fewer complications, little discomfort. Prospective and comparative study is recommended to evaluate this procedure and traditional method.
