HIF-1α、CAⅨ和Glut-1在宫颈癌组织中的表达及意义

摘　　要：目的探讨缺氧诱导因子1α(HIF-1α)、碳酸酐酶Ⅸ(CAⅨ)及葡萄糖转运蛋白1(Glut-1)在宫颈癌发生、发展中的作用。方法采用免疫组织化学SP法检测宫颈鳞状细胞癌(宫颈癌组,64例)、宫颈上皮内瘤变(CIN组,57例)和正常宫颈黏膜上皮组织(对照组,26例)中HIF-1α、CAⅨ及Glut-1的表达,分析其与宫颈癌临床病理特征和预后之间的关系。结果宫颈癌组、CIN组和对照组HIF-1α阳性表达率分别为73.44%、43.86%和7.69%(P<0.05),CAⅨ阳性表达率分别为78.13%、54.39%和11.54%(P<0.05),Glut-1阳性表达率分别为51.56%、26.32%和0(P<0.05)。宫颈癌组织中HIF-1α阳性表达与肿瘤组织学分级及临床分期相关(P<0.05);CAⅨ阳性表达与肿瘤组织学分级和淋巴结转移相关(P<0.05);Glut-1阳性表达与肿瘤大小及患者年龄相关(P<0.05)。宫颈癌组织中HIF-1α与CAⅨ的阳性表达呈正相关(r=0.400,P<0.05),HIF-1α与Glut-1的阳性表达呈正相关(r=0.693,P<0.05)。HIF-1α与CAⅨ阳性表达宫颈癌患者的生存率低于阴性表达者(P<0.05)。结论HIF-1α、CAⅨ和Glut-1蛋白在宫颈癌组织中均呈过表达;HIF-1α蛋白的阳性表达在宫颈癌诊断及分期中有一定价值;宫颈癌组织中HIF-1α和CAⅨ蛋白阳性表达与患者较差的预后密切相关。Objective To investigate the expressions and clinical significance of hypoxiainducible factor 1α（HIF-1α）,carbonic anhydrase Ⅸ（CAⅨ）and glucose transporter 1（Glut-1）in cervical carcinoma.Methods The expressions of HIF-1α,CAⅨ and Glut-1protein were detected in cervical squamous cell carcinoma tissues（group SCC,64 cases）,cervical intraepithelial neoplasia tissues（group CIN,57 cases）and normal cervical epithelial tissues（group NCE,26 cases）by immunohistochemisty.The correlation between the expressions of HIF-1α,CAⅨ and Glut-1with the clinicopathological features and the prognosis of patients with cervical carcinoma was analyzed.Results For groups of SCC,CIN and NCE,the positive expression rates of HIF-1αwere 73.44%,43.86%and7.69%（P〈0.05）,those of CAⅨ were 78.13%,54.39% and 11.54%（P〈0.05）,and those of Glut-1were 51.56%,26.32%and 0（P〈0.05）.The positive expression of HIF-1αin group SCC was closely associated with the differentiation and FIGO stage of tumor,which of CAⅨ was associated with differentiation and lymph node metastasis of tumor,and of Glut-1was associated with the age and tumor size（P〈0.05）.The expression of HIF-1α was positively correlated to CAⅨ（r=0.400,P〈0.05）.So did HIF-1αand Glut-1（r=0.693,P〈0.05）in group SCC.Survival rate in the patients with positive expressions of HIF-1αand CAⅨ was significantly lower than that in those with HIF-1αand CAⅨ negative（P〈0.05）.Conclusion The overexpressions of HIF-1α,CAⅨ and Glut-1protein are common,and have a certain value in the diagnosis,tumor staging and predicting poor prognosis in the patients with cervical carcinoma.
