新生儿高TSH血症的转归及体格智能发育研究

摘　　要：目的：探讨高TSH血症患儿的转归及其影响因素和不同转归下的体格智能发育,为临床治疗、随访提供参考依据。方法：2012~2014年筛查并诊断为高TSH血症的新生儿,第一次抽血复查TSH≥15 m IU/L者进行药物治疗,TSH在6.27~15 m IU/L之间者不予服药,进行定期随访至1岁,3月龄以内者每月随访一次,3~12月龄每2~3个月随访一次。结果：随访的86例高TSH血症患儿中有82.6%的患儿甲状腺功能可在半岁以内完全自行恢复正常。随访组、治疗组与正常对照组相比,患儿近一岁时测评贝利智能MDI、PDI进行方差分析：F值分别为0.521、0.270,P值均小于0.05无统计学差异。三组身长发育亦无显著性差异,x2=3.07,P〈0.05。结论：低值高TSH血症患儿甲状腺功能大部分可自行恢复正常,且体格智能发育与同龄儿童无显著性差异,故对低值高TSH血症的治疗要慎重,避免过度治疗。Objective： Explore the outcome and factors of children with hyperthyrotropinemia and its physical and intellectual development under different outcome for providing a reference of clinical treatment and follow - up. Method ： Neonates were diagnosed hyperthyrotropinemia in 2012--2014 years. Some were treated with drug whose concentration of thyrotropin were greater than 15 mU/ L . The others whose concentration of thyrotropin were 6.27-- 15 mlU/L, were regularly followed up to one year of age ：once a month flow - up within 3 months of age; once every 2 -- 3 months flow - up between 3 - 12 months of age. Result ： Thyroid function of 82.6 % children with hyperthyrotropinemia can be completely return to normal within six months in follow- up of 86 cases. Follow- up group compared to the control group and treatment group, Children at the age of nearly one evaluate Bailey intelligent MDI, PDI analyzed by ANOVA ： F values were 0. 521,0. 270, P values less than 0.05 which were not statistically significant. High development of the three groups had no significant difference, x2 = 3.07, P 〈 0.05. Conclusion： Low thyroid function in children with hyperthyrotropinemia most can restore itself to normal, and physical and mental development of children of the three groups had no significant difference . So the treatment of hyperthyrotropinemia should be careful to avoid over- treatment when TSH values were low.
