摘 要:目的探讨HIF-1α、VEGF、MVD(用抗CD34抗体标记)在大肠腺癌组织中的表达及与临床病理特征的关系。方法采用免疫组织化学S-P法检测广东医学院附属医院经病理检查证实的手术切除标本或活检组织中HIF-1α、VEGF、MVD的表达情况,其中38例大肠腺癌组织、22例大肠腺瘤组织、29例正常大肠组织。结果 HIF-1α、VEGF在大肠腺癌组的阳性表达率分别为73.7%、65.8%,明显高于大肠腺瘤组、正常大肠组,差异有统计学意义(P<0.05)。大肠腺癌、大肠腺瘤、正常大肠组织中MVD值分别是(52.36±9.66)、(30.23±5.22)、(21.37±4.755),MVD值在腺癌中明显高于腺瘤及正常肠组织(P<0.05)。28例HIF-1α呈阳性表达的大肠腺癌组织MVD值为(53.22±10.15);呈阴性表达的10例中MVD值为(42.75±10.47),呈正相关关系(rs=0.392,P<0.05)。VEGF呈阳性表达的25例大肠腺癌组织MVD值为(54.54±9.60);呈阴性表达的10例中MVD值为(42.63±9.80),呈正相关关系(rs=0.491,P<0.05)。结论在大肠腺癌组织中HIF-1α和VEGF的表达明显高于大肠腺瘤组织和正常大肠组织,提示HIF-1α和VEGF可能参与大肠腺癌的发展。MVD值在大肠腺癌中的表达明显高于大肠腺瘤组织和正常大肠组织,提示大肠腺癌中血管生成较大肠腺瘤组织和正常大肠组织丰富;MVD值是衡量大肠腺癌发展、浸润及转移的重要指标。HIF-1α、VEGF的表达与MVD值在大肠腺癌组织中均呈正相关,提示HIF-1α、VEGF可能与大癌发展过程中血管生成密切相关。Objective To investigate the expression of HIF-la, VEGF, MVD (anti CD34 antibody) in colorectal carcinoma and their relationship with clinic pathological features. Methods An immunohistochemical method (steptavidin-biotin peroxldase, SP) was used to detect the expressions of HIF-1 α , VEGF and MVD in 38 cases of Colorectal Carcinoma tissues, 22 cases of colorectal adenoma tissues and 28 cases of normal colorectal tissues, all patients were from the Affiliated Hospital of Guangdong Medical College. Results The positive expression rates of HIF-1 and VEGF in colorectal adenoearcinoma group were 73.7% and 65.8% respectively, which were significantly higher than those in the colorectal adenoma group and the normal group, the difference was statistically significant (P 〈 0.05).Colorectal carcinoma, colorectal adenoma and normal colorectal tissue MVD values were(52.36 ~ 9.66),(30. 23 ~ 5.22),(21.37 ~ 4.755),the value of MVD in carcinoma was significantly higher than that in adenoma and normal eolorectal tissues (P 〈 0.05). The MVD value(53.22 ~ 10.15)in 28 case colorectal carcinoma with positive HIF-1 ct expression was significantly higher than that with negative HIF-1 α expression (42.75 - 10.47). Correlation analysis showed that MVD value had a positive correlation with HIF-1 α(rs=0.392,P 〈 0.05).The MVD value ( 54.54 +- 9.60 ) in 25 case colorectal carcinoma with positive VEGF expression was significantly higher than that with negative VEGF expression (42.63 ~ 9.80).Correlation analysis showed that MVD value had a positive correlation with VEGF (rs=0.491, P 〈 0.05). Conclusion The expression of HIF-1α and VEGF in colorectal carcinoma is higher than that in colorectal adenoma tissues and normal colorectal tissues. It suggests that HIF-1 α and VEGF may play an important role in the tumor genesis and progression of colorectal carcinoma.
