不同治疗方式治疗乳腺癌患者术后皮瓣下积液伴感染的效果分析

摘　　要：目的探讨临床中不同的治疗方式在乳腺癌术后皮瓣下积液合并感染治疗中的应用效果,为临床治疗该病提供参考。方法选取医院2012年1月-2015年12月50例乳腺癌术后皮瓣下积液合并感染患者作为本研究对象,将患者按照不同的治疗方法分为A组和B组,每组各25例;A组给予引流管连接负压瓶引流处理,B组给予纱布填塞引流处理,观察两组患者的临床治疗效果。结果 A组患者在皮瓣愈合时间、换药次数、住院时间均明显低于B组,两组差异有统计学意义(P<0.05);A组患者在生活质量评分明显优于B组,两组差异有统计学意义(P<0.05);治疗后两组患者的C-反应蛋白(CRP)和白细胞(WBC)计数均较治疗前明显降低,且A组降低程度优于B组,差异有统计学意义(P<0.05)。结论临床中对于乳腺癌术后积液合并感染的患者给予引流管连接负压瓶引流处理的效果优于纱布填塞引流,且缩短患者皮瓣愈合时间、住院时间和减少换药次数,提高患者的生活质量,值得临床推广应用。OBJECTIVE To explore the clinical effects of different approaches on treatment of postoperative flap effusion complicated with infections so as to provide guidance for clinical treatment of the disease.METHODS A total of 50 breast cancer patients complicated with postoperative flap effusion and infections who were treated in hospitals from Jan 2012 to Dec 2015 were recruited as the study objects and divided into the group A and the group B according to the treatment approach,with 25 cases in each group.The group A was treated with drainage tube connecting with negative pressure bottle,the group B was treated with gauze packing drainage.The clinical curative effects were observed and compared between the two groups of patients.RESULTS The flap healing time,dressing change times,and length of hospital stay were significantly lower in the group A than in the group B（P〈0.05）.The life quality score of the group A was significantly better than that of the group B（P〈0.05）.The levels of C-reactive protein（CRP）and white blood cell（WBC）counts of the two groups of patients were significantly lower after the treatment than before the treatment and were reduced significently in the group A than in the group B（P〈0.05）.CONCLUSIONThe drainage with tube connecting with vacuum bottle is superior to the gauze packing drainage on the clinical treatment of the breast cancer patients complicated with postoperative effusion and infections,it can shorten the flap healing time and length of hospital stay,reduce the number of times of dressing change,and improve the quality of life of the patients,which is worthy to be promoted in the hospital.
