摘  要:目的:探讨非均匀性脂肪肝超声图像特点,为超声技术对脂肪肝的诊断与鉴别提供依据。方法:回顾性分析77例确诊为非均匀性脂肪肝患者的声像图。结果:根据声像图表现,将非均匀性脂肪肝分为三型:弥漫非均匀浸润型(58.4%):大部分肝实质被脂肪浸润,残存小片状正常区为低回声。叶段浸润型(24.8%):致密高回声位于肝叶或肝段。局限浸润型(16.9%):显示为肝内局限性单个或多个小片状高回声区。彩色多普勒超声(CDFI)显示:异常回声区周围无血流环绕,内部无穿支样血流,部分异常回声区内显示与正常肝静脉或门静脉相延续的血流信号。结论:非均匀性脂肪肝的超声诊断有其声像图特点,可以较准确地进行诊断及鉴别诊断。Objective:To discuss the ultrasonic image characteristics of non-uniform fatty liver.Methods:Retrospective analysis of sonograms from 77 patients confirmed with non-uniform fatty liver disease.Results:According to the imaging,the non-uniform fatty liver was divided into three types:Diffuse non-uniform infiltration type(58.4%) , in which most of the liver parenchyma were infiltrated with fat,and the remaining small pieces of normal area were hypoechoic;Lobe segment infiltration type(24.8%),in which the high density and high echo were located in the hepatic lobe or segment;Finite infiltration type (16.9%),which showed localized small piece-shaped high echo areas inside the liver.Color Doppler Flow Imaging ( CDFI) showed there was no blood flow around the abnormal echo areas and no internal perforator like blood flow.Some abnormal echo areas showed continuous blood flow signal corresponding with normal he-patic veins or portal veins.Conclusion:The ultrasonic diagnosis of non-uniform fatty liver disease has sonogram features,which can be used in the diagnosis and differential diagnosis.
