Habib 4X在腹腔镜肝切除术中的应用

摘　　要：目的 ：探讨H abib 4X在腹腔镜肝切除术（LH）中的应用价值。方法：回顾性分析2014年6—12月厦门大学附属成功医院肝胆外科收治的14例因肝脏疾病行LH的患者的临床资料均采用H abibTM4X凝固电极（美国A ngio D ynam ics R ITA医疗系统）对肝内瘤体行重叠射频消融术,观察术中出血量、手术时间、输血情况、住院时间、术后肝功能恢复情况、术后并发症发生情况。结果：14例患者均在未阻断肝门血管情况下成功完成LH。术中及术后均未行输血治疗,术中平均出血122.86 m L（150~240m L）;平均手术时间122.29 m in（83~160 m in）;术后平均住院天数9.57 d（7~13 d）。所有患者术后恢复良好,术后未发生出血、胆汁漏、肝功能衰竭。结论：在H abib 4X辅助下可安全、有效的行腹腔镜解剖性和非解剖性肝切除,具有一定的临床应用价值。Objective： To investigate the clinic value of the bipolar radiofrequency device （la- paroscopic Habib 4X） for Laparoscopic hepatectomy. Methods： From June 2014 to December 2014,14 patients with liver disease who received Laparoscopic hepatectomy with laparoscopic Habib 4X at the Affiliated Chenggong Hospital of Xiamen University were retrospectively analyzed. Habib 4X bipolar radiofrequency devices （USA AngioDynamics RITA Medical Systems） were over- lapped for the intrahepatic tumor. Intraoperative blood loss, operative time, blood transfusion, post- operative hospital stay, postoperative liver function, postoperative complications were observed. Results： The 14 cases were successfully received Laparoscopic hepatectomy without blocking the hepatic portal vascular. None of the patients were received intraoperative and postoperative bloodtransfusion therapy, the mean intraoperative blood loss volume was 122.86 （150~240） mL, the mean operative time was 122.29 min（83~160 min）, the mean postoperative hospital stay was 9.57 （7N13） d. All patients recovered well, no postoperative bleeding, bile leak, liver failure. Conclusion： The Laparoscopic Habib 4X bipolar radiofrequency device can be safely and effectively performed laparo- scopic dissection and non-anatomical liver resection, which has an important clinical value for the Laparoscopic hepatectomy.
