胃癌病变中Runx3基因甲基化与幽门螺杆菌感染的关系

摘　　要：目的:研究幽门螺杆菌(helicobacterpylori,Hp)感染、抑癌基因Runx3基因5'-Cp G岛甲基化与胃癌的相关性。方法:收集2014年3月至2015年9月包头医学院第一附属医院消化内科门诊及住院患者56例经胃镜及病理确诊为胃癌者,70例对照组选自同期该院消化内科经胃镜及病理诊为慢性浅表性胃炎患者。所有受检者行胃镜前均行C13尿素呼气试验检测Hp感染情况。采用甲基化特异性聚合酶链式反应法检测胃癌组及对照组中Runx3基因的甲基化状态。结果:胃癌组Runx3基因甲基化率(53.6%,30/56)高于对照组(7.1%,5/70)(P<0.05)。胃癌组与对照组Hp感染差异有统计学意义(P<0.05)。Hp阳性胃癌组与Hp阴性胃癌组比较,Runx3基因甲基化差异有统计学意义(P<0.05)。结论:Runx3基因甲基化与胃癌发生有关,幽门螺杆菌感染增加抑癌基因Runx3基因5'Cp G岛出现甲基化,导致胃癌的发生。Objective： To research the correlation of helicobacter pylori（ Hp） infection and methylation in the 5 ＇Cp G island of Runx3 tumor suppressor gene with gastric cancer. Methods： 56 cases of gastric cancer patients diagnosed and confirmed by endoscopy and pathology in Gastroenterology Department of the First Affiliated Hospital of Baotou Medical College from March 2014 to September 2015 were selected as gastric cancer group. 70 cases of chronic superficial gastritis patients during the same period in the hospital were selected as control group. C13 urea breath test was adopted to test Hp infection of all subjects prior to gastroscope. Methylation status of Runx3 gene was detected in the gastric cancer group and control group by Methylation- specific PCR（ MSP）. Results： Runx3 gene methylation rate in the gastric cancer group（ 53. 6 %,30 /56） was higher than that in the control group（ 7. 1 %,5 /70） and the difference was statistically significant（ P〈0. 05）. There was significant difference in helicobacter pylori infection between the gastric cancer group and the control group（ P〈0. 05）. There was significant difference in Runx3 gene methylation between Hp- negative gastric cancer group and Hp- positive gastric cancer group（ P〈0. 05）. Conclusion： Runx3 gene methylation may be related to the incidence of gastric cancer. Helicobacter pylori infection increases the methylation of 5＇Cp G island of Runx3 tumor suppressor gene,leading to gastric cancer.
