CML慢性期患者血管VEGF、Flt-1的表达水平及其临床意义

摘　　要：[目的]探讨慢性粒细胞白血病（CML）慢性期患者血管内皮生长因子（VEGF）及其受体Fit-1的表达水平及其临床意义。[方法]选择2013年8月至2015年11月于本院就诊的CML患者45例作为研究对象（观察组），选取同期在本院健康体检正常的40例作为对照（对照组），比较两组血清和骨髓单个核细胞（BMMCs）中VEGF和Fit-1相对表达量和BMMCs中VEGF蛋白和VEGF-mRNA表达阳性率的差异，分析CML患者治疗疗效、预后与血清和BMMCs中VEGF表达的关系。[结果]观察组血清和BMMCs培养上清液中的VEGF、Fit-1表达量高于对照组，差异具有统计学意义（P〈0．05）；观察组VEGF蛋白和VEGF-mRNA表达阳性率均高于对照组，差异具有统计学意义（P〈0.05）；治疗有效的患者血清中VEGF表达量低于治疗前和治疗无效组，且差异具有统计学意义（P〈0．05）；观察组BMMCs中VEGF蛋白表达阳性的患者慢性期持续时间（中位时间为27个月）明显低于VEGF蛋白表达阴性的患者（中位时间为36个月），且差异具有统计学意义（P〈0．05）。[结论]CML慢性期的患者VEGF、Flt-1的表达水平明显增高，且与患者的治疗疗效和预后密切相关。[Objective] To investigate the expression level and clinical significance of VEGF and Fh-1 in patients with chronic myeloid leukemia （CML） of chronic phase.[Methods] Forty-five cases of chronic myeloid leukemia in our hospital from August 2013 to November 2015 were collected as observation group, and 40 cases of health exami- nation were collected in the same period as normal/control group. Relative expression levels of VEGF and Flt-1 in ser- um and bone marrow mononuclear cells （BMMCs） and the positive expression rate of VEGF protein and VEGF-mR- NA in BMMCsin of the two groups were compared. The relationship between the therapeutic effect and prognosis of CML patients and the expression of VEGF in serum and BMMCs were evaluated.[Results] The expression of VEGF and Flt-1 in the serum and BMMCs culture supernatant in the observation group was higher than that in the control group, and the difference was statistically significant （ P〈0.05） ; The positive expression rates of VEGF protein and VEGF mRNA in the observation group were higher than those in the control group, the difference was statistically significant （ P〈0.05） ; The expression of VEGF in the serum of effective patients was lower than that before treat- ment and that of ineffective group, and the difference was statistically significant （ P〈0.05）; The duration of the chronic phase of VEGF protein positive expression patients in BMMCs of the observation group（median time was 27 months） was significantly shorter than that of the VEGF protein negative expression patients （median time was 36 months） ,and the difference was statistically significant （ P〈0.05）.[Conclusion] The expression level of VEGF and Fh-1 is significantly increased in patients with CML in chronic phase, and it is closely related to the therapeutic effect and prognosis.
