摘  要:[目的]探讨脑池积血对动脉瘤性蛛网膜下腔出血(aSAH)患者术后发生急性脑积水的影响。[方法]回顾性分析2013年6月至2015年6月在本院接受手术治疗的200例aSAH患者,将其分为急性脑积水组和非脑积水组,每组各100例,对脑积水和非脑积水组患者相关临床资料进行单因素和多N素Logistic回归分析。[结果]急性脑积水组和非脑积水组患者相关临床资料单因素分析显示,脑池积血、Fisher分级、治疗方式、Hunt-Hess分级等比较,差异具有统计学意义(均P〈0.05)。多因素Logistic回归分析显示,脑池积血、Fisher分级为aSAH后急性脑积水相关危险因素,差异均具有统计学意义(均P〈0.05)。[结论]导致asAH患者术后发生急性脑积水有多种因素,脑池积血、Fisher分级是aSAH后脑积水发生的独立危险因素。[Objective]To investigate effect of cisternal hemorrhage on acute hydrocephalus in patients with aneurysmal subarachnoid hemorrhage (aSAH) after surgery. [Methods]Retrospective analysis of 200 patients with aSAH who underwent surgical treatment in our hospital from June 2015 to June 2013 was conducted, and the pa- tients were divided into acute hydrocephalus group and non hydrocephalus group, 100 in each group, Single factor and multi factor Logistic regression analysis were performed on the clinical data of patients with hydrocephalus and non hydrocepha!us.[Results]Univariate analysis of clinical data of patients with acute hydrocephalus and non hy- drocephalus showed that in the comparison of cisternal hemorrhage, Fisher classification, treatment, Hunt-Hess classification and so on , the difference was statistically significant (mean P ~0.05). Multi factor Logistic regres- sion analysis demonstrated cisternal hemorrhage , Fisher classification were the risk factors of acute hydrocephalus after aSAH, the difference was statistically significant (mean P 〈0.05).[Conelusion]There are many factors that lead to the oecurreace of acute hydrocephalus after aSAH. Cisternal hemorrhage and Fisher classification are the independent risk factors for the occurrence of hydrocephalus after aSAH.
