儿童肾透明细胞肉瘤的CT表现

摘　　要：目的 探讨肾透明细胞肉瘤（CCSK）的CT表现及病理特点，提高对此病的认识。方法 回顾性分析经手术病理证实的CCSK患者8例。结合CT平扫、多期增强扫描图像和术后病理免疫组化等资料，总结其特征性表现。结果 右肾肿块3例，左肾肿块5例，直径均＞8 cm，表现为类圆形软组织肿块从肾门区向外单中心膨胀性生长，肾集合系统受压变形，残肾组织受压呈杯口状改变，肿瘤累及肾髓质，包膜完整，边界清晰；病变密度较均匀，以实性病变为主，其中2例见小斑片状出血灶，均无淋巴结转移及瘤栓形成。增强扫描肿瘤明显不均匀强化，实性成分呈云絮样、鱼肉样或虎斑样明显强化，瘤内可见多发斑点状及条纹状高密度血管影。结论 CCSK的CT表现具有一定的特征性，有助于临床诊断及鉴别诊断。Objective To analyze the CT manifestations and pathologic characteristics of clear cell sarcoma of the kidney（CCSK）, to improve the realization of this disease entity. Methods 8 cases with CCSK confirmed by operation and pathology were analyzed retrospectively. Combined with CT findings and clinical data, the features of CCSK were summarized. Results The tumors involved right kidney in 3 cases, left kidney in 5 cases,and the diameters of all lesions were more than 8 cm. The tumors were originated from the renal hilum with expansion growth type, accompanied by deformation of renal collective system. The residual renal tissue showed cup-shaped. All tumors had intact membrane and clear boundaries. The lesions showed homogenous density and 2 cases had small patchy hemorrhage. Lymph node metastasis and tumor ernbolus were not seen. Solid component presented markedly inhomogeneous enhancement, and showed tumor vessels with high density. Conclusion The CT manifestations of CCSK are characteristic, which provided helpful information in clinical diagnosis.
