不同手术方式治疗青光眼术后硬核白内障疗效观察

摘　　要：目的观察巩膜隧道小切口超声劈核后手法娩出核与透明角膜切口超声乳化碎核治疗青光眼术后硬核白内障的疗效和安全性，为临床选择合适的手术方法提供依据。方法将118例（130眼）青光眼术后硬核白内障患者按照随机数字表法分为两组，每组59例（65眼）。A组采用巩膜隧道小切口超声劈核后手法娩出核治疗，B组采用透明角膜切口超声乳化碎核治疗。比较两组治疗前后的视力情况、前房深度分级、前后眼压及角膜内皮细胞密度，统计并发症发生情况。结果治疗前两组各项指标比较差异均无显著性（P〉0．05）；治疗后两组视力、前房深度分级、角膜内皮细胞计数均较治疗前显著改善（P〈0．01），眼压无显著变化（P〉0．05），两组间前房深度分级、角膜内皮细胞计数比较差异有显著性（P〈0．01），视力、眼压比较差异无显著性（P〉0．05）。A组并发症发生率为32．3％，B组为26．2％，两组比较差异无显著性（P〉0．05）。结论采用巩膜隧道小切口超声劈核后手法娩出核与透明角膜切口超声乳化碎核治疗青光眼术后硬核白内障疗效显著，安全性高，但前者在改善前房深度分级方面具有明显优势。Objective To observe the curative effect and safety of manual small incision cataract surgery （MSICS） and phacoemulsification for hard cataract following glaucoma surgery in order to provide basis for selecting proper operation method clinically. Methods A total of 118 patients with cataract following glaucoma surgery （130 eyes） were assigned to two groups of 59 ones （65 eyes） each. Group A received the MSICS and B did phacoemulsification. Pre and post-treatment visions, anterior chamber depth grades, anterior-posterior intraocular pressure （lOP） and corneal endothelial cell density were compared between two groups, complications added up. Results There were no significant group differences in all indexes before treatment （P〉0.05）; after treatment visions, anterior chamber depth grades and corneal endothelial cell counts of both groups improved more significantly （P〈0.01）, the IOP had no significant changes （P〉0.05）, there were significant group differences in anterior chamber depth grades and corneal endothelial cell counts （P〈0.01）, and visions and the IOP had no significant group differences （P〉0.05）. Complicationrate was respectively 32.3% in group A and 26.2% in B, which showed no significant group differ- ence （P〉0.05）. Conclusion Both the MSICS and Dhacoemulsification for hard cataract following glaucoma surgery have an evident effect and higher safety, but the former has an advantage in improvinganterior chamber depth grades.
